- Adding
insult to injury should never be the result of our treatment.
Increasing our learning curve of surgeons should never be done at the
expense of our inexperience.
- This is a difficult case of a 38
year-old male heavy manual worker. Two years ago he suffered an engaged
anterior dislocation which was initially treated by a a traditional
healer with no success. After one year after going from here to there,
he finally was treated surgically.
- Unfortunately, the result
was suboptimal and the open reduction was insufficient, and due to this
fact, the surgeons decided to place two transfictive pins damaging the
articular surface of humerus and glenoid. Six months postoP the patient
developed avascular necrosis. Awaiting the final treatment he suffered a
3 meter fall (he still needs to work) and suffered a 2-part proximal
humerus fracture.
- I decided to go with an open reduction of the
joint, cerclage of the fracture, hemiarthroplasty due to his work, and
ream and run on the glenoid side.
- Could have done better? Not sure about it. What are your thoughts?

- Este es un caso
difícil de un trabajador manual pesado de 38 años. Hace dos años sufrió
una luxación anterior comprometida que inicialmente fue tratada por un
curandero sin éxito. Después de un año de ir de aquí para allá,
finalmente fue tratado quirúrgicamente.
- Desafortunadamente, el
resultado fue sub-óptimo y la reducción abierta fue insuficiente, por lo
que los cirujanos decidieron colocar dos pines transfictivos dañando la
superficie articular del húmero y la glenoides.
- Seis meses
después de la operación, el paciente desarrolló necrosis avascular. En
espera del tratamiento final, sufrió una caída de 3 metros (todavía
necesita trabajar) y sufrió una fractura de húmero proximal en dos
partes.
- Decidí ir con una reducción abierta de la articulación,
cerclaje de la fractura, hemiartroplastia debido a su trabajo, y fresado
y corrido en el lado glenoideo.
