Abstract
BACKGROUND AND OBJECTIVES: Candidates for renal transplantation are at increased risk for complications related to cardiovascular disease; however, the optimal strategy to reduce this risk is not clear. The aim of this study was to evaluate the variability among existing guidelines forpreoperative cardiac evaluation of renal transplant candidates. DESIGN, SETTING, PARTICIPANTS, & MEASUREMENTS: A consecutive series of renal transplant candidates (n=204) were identified, and four prominent preoperative cardiac evaluation guidelines, pertaining to this population, were retrospectively applied to determine the rate at which each guideline recommended cardiac stress testing. RESULTS: The rate of pretransplant cardiac stress testing would have ranged from 20 to 100% depending on which guideline was applied. The American Heart Association/American College of Cardiology (ACC/AHA) guideline resulted in the lowest rate of testing (20%). In our population, 178 study subjects underwent stress testing: 17 were found to have ischemia and 10 underwent revascularization. The ACC/AHA approach would have decreased the number of noninvasive tests from 178 to 39; it would have identified only 4 of the 10 patients who underwent revascularization. The three other guidelines (renal transplant-specific guidelines) recommended widespread pretransplant cardiac testing and thus identified nearly allpatients who had ischemia on stress testing. CONCLUSIONS: The ACC/AHA perioperative guideline may be inadequate for identifying renal transplant candidates with coronary disease; however, renal transplant-specific guidelines may provoke significant overtesting. An intermediate approach based on risk factors specific to the ESRD population may optimize detection of coronary disease and limit testing.

Abstract
Kidney transplantation is the treatment of choice for most patients with stage 5 chronic kidney disease and end-stage renal disease (ESRD), offering improved quality of life and overall survival rates. However, the limited supply of available organs makes this a scarce resource. Cardiovascular complications continue to be the leading cause of mortality in the kidney transplant population, accounting for over 30% of deaths with a functioning allograft. Thus, preoperative cardiac risk assessment is critical to optimize patient selection and outcomes. Currently there is no consensus for cardiovascular evaluation in the chronic kidney disease and ESRD population prior to kidney transplantation; the recommendations of the American Society of Nephrology and American Society of Transplantation differ from those of the American Heart Association and the American College of Cardiology. Previously developed risk scores have also been used to risk stratify this population. In this review, we discuss two cases that illustrate the difficulties of interpreting the prognostic value of current testing strategies. We also discuss the importance of different tests for cardiovascularevaluation as well as previous nonkidney transplant specific risk scores used in the pre-kidney transplant population.

Abstract
Patients with chronic renal failure (CRF) due to various mechanisms are prone to significant pulmonary comorbidities. With the improvements in renal replacement therapy (RRT), patients with CRF are now expected to live longer, and thus may develop complications in the lung from these processes. The preferred treatment of CRF is kidney transplantation and patients who are selected to undergo transplant must have a thorough preoperative pulmonary evaluation to assess pulmonary status and to determine risk of postoperative pulmonary complications. A MEDLINE ® /PubMed ® search was performed to identify all articles outlining the course of pre-surgical pulmonary evaluation with an emphasis on patients with CRF who have been selected for renal transplant. Literature review concluded that in addition to generic pre-surgical evaluation, renal transplant patients must also undergo a full cardiopulmonary and sleep evaluation to investigate possible existing pulmonary pathologies. Presence of any risk factor should then be aggressively managed or treated prior to surgery.

Coronary artery disease in patients with chronic kidney disease: a clinical update.

Abstract
Chronic kidney disease (CKD) is an independent risk factor for coronary artery disease (CAD). Coronary artery disease is the leading cause of morbidity and mortality in patients with CKD. The outcomes of CAD are poorer in patients with CKD. In addition to traditional risk factors, several uremia-related risk factors such as inflammation, oxidative stress, endothelial dysfunction, coronary artery calcification, hyperhomocysteinemia, and immunosuppressants have been associated with accelerated atherosclerosis. A number of uremia-related biomarkers are identified as predictors of cardiac outcomes in CKD patients. The symptoms of CAD may not be typical in patients with CKD. Both dobutamine stress echocardiography and radionuclide myocardial perfusion imaging have moderate sensitivity and specificity in detecting obstructive CAD in CKD patients. Invasive coronary angiography carries a risk of contrast nephropathy in patients with advanced CKD. It should be reserved for those patients with a high risk for CAD and those who would benefit from revascularization. Guideline-recommended therapies are, in general, underutilized in renal patients. Medical therapy should be considered the initial strategy for clinically stable CAD. The effects of statins in patients with advanced CKD have been neutral despite a lipid-lowering effect. Compared to non-CKD population, percutaneous coronary intervention (PCI) is associated with higher procedure complications, restenosis, and future cardiac events even in the drug-eluting stent era in patients with CKD. Compared with PCI, coronary artery bypass grafting (CABG) reduces repeat revascularizations but is associated with significant perioperative morbidity and mortality. Screening for CAD is an important part of preoperative evaluation for kidney transplant candidates.

Abstract
INTRODUCTION: Pilot study associating hemodialysis-to-general-anesthesia time interval and post-operative complications in hemodialysis patientsto better define a more optimal pre-anesthetic waiting period. METHODS: Pre-anesthetic and 48-hours post-anesthetic parameters (age, gender, body-mass-index, pre-operative ultrafiltrate, potassium, renaldisease etiology, hemodialysis sessions per week, Acute Physiology and Chronic Health Evaluation-II score, Portsmouth-Physiologic and Operative Severity Score for the Enumeration of Mortality and Morbidity, American Society of Anesthesiologists physical status, Johns Hopkins Surgical Classification System Category, surgical urgency, intra-operative fluids, estimated blood loss, post-operative complications) were collected on chronic hemodialysis patients between 11/2009-12/2010. Continuous data were analyzed by Analysis of Variance or t-test. Bivariate data were analyzed by Fisher's Exact Test. Relative Risks/Confidence Intervals were calculated for statistically significant comparisons (p=0.05). Exclusion criteria were incomplete records, peritoneal dialysis, intra-operative hemodialysis, liver transplant, and cardiopulmonary bypass. RESULTS: Patients were grouped by dialysis to anesthesia time interval: Group 1 >24 hours, Group 2 7-23.9 hours, Group 3 < 7 hours. Among Surgical Category 3-5 patients, hypotension was more common in Group 3 than Group 1 (63.6% vs 9.2%, p<0.0001, relative risk=6.9, confidence interval=3.0-15.7) or Group 2 (63.6% vs 17.3%, p=0.0002, relative risk=3.7, confidence interval=1.9-7.2). Other complications rates were not statistically significant. Disease and surgical severity scores, preoperative ultrafiltrate, and intra-operative fluids were not different. CONCLUSIONS:
Post-anesthetic hypotension within 48 hours was more common in those with < 7 hours interval between dialysis and anesthesia. Therefore, if surgical urgency permits, a delay of ≥7 hours may limit postoperative hypotension. More precise associations should be obtained through a prospective study.

Abstract
INTRODUCTION:The difficulties in the management of the blunt chest wall trauma patient in the Emergency Department due to the development of late complications are well recognised in the literature. Pre-injury anti-platelet therapy has been previously investigated as a risk factor for poor outcomes following traumatic head injury, but not in the blunt chest wall trauma patient cohort. The aim of this study was to investigate pre-injury anti-platelet therapy as a risk factor for the development of complications in the recovery phase following blunt chest wall trauma. METHODS:A retrospective study was completed in which the medical notes were analysed of all blunt chest wall trauma patients presenting to a large trauma centre in Wales in 2012 and 2013. Using univariate and multivariable logistic regression analysis, pre-injury platelet therapy was investigated as a risk factor for the development of complications following blunt chest wall trauma. Previously identified risk factors were included in the analysis to address the influence of confounding. RESULTS: A total of 1303 isolated blunt chest wall trauma patients presented to the ED in Morriston Hospital in 2012 and 2013 with complications recorded in 144 patients (11%). On multi-variable analysis, pre-injury anti-platelet therapy was found to be a significant risk factor for the development of complications following isolated blunt chest wall trauma (odds ratio: 16.9; 95% confidence intervals: 8.2-35.2). As in previous studies patient age, number of rib fractures, chronic lung disease and pre-injury anti-coagulant use were also found to be significant risk factors. CONCLUSIONS: Pre-injury anti-platelet therapy is being increasingly used as a first line treatment for a number of conditions and there is a concurrent increase in trauma in the elderly population. Pre-injury anti-platelet therapy should be considered as a risk factor for the development of complications by clinicians managing blunt chest wall trauma.

Abstract

INTRODUCTION: horax injuries are to be found in approximately 78% amongst all accident victims. Moreover, they implicate an increase in mortality rate. Consequently, an adequate contemporary treatment has to begin preclinically, even if the conditions are less comfortable than in a clinical setting. Emergency doctors need to be familiar with the placement of chest tubes. MATERIALS AND METHODS: From January 1, 2007 to December 31, 2010, emergency doctors of the rescue helicopter site Christoph 20 had to place chest tubes directly at the scene of an accident in 49 patients. These patients were now reidentified, and their clinical course was reevaluated. By means of apparative diagnostics, it was possible to analyze the location of the tubes tip. Following a comparison of the patient, outcome versus the quality of preclinical thoracic discharge could be made. RESULTS: The preclinical placement of a chest tube became necessary mainly because of a blunt thoracic trauma. This was predominantly related to victims of traffic accidents, whereas male victims clearly dominated. Forty-two of those patients received further treatment at the Klinikum Bayreuth hospital, enabling an analysis of the tubes location by CAT (computed axial tomography) scan. Six patients had been discharged on both sides, contributing to 48 tube tips that could be examined concerning their location. Of the 48 chest tubes, 46 had been placed from a lateral approach. The ventral access by Monaldi had only been chosen in two cases. Altogether, nine incorrect placements, mainly within the right interlobe gap, had been detected. CONCLUSIONS: The study collective showed a significant preference to the lateral approach when placing a chest tube at the emergency scene of an accident. In total, a prevalence of 19% incorrect placements could be revealed, meaning the chest tube had either been placed within the lung parenchyma, the interlobe gap, or extrathoracically. Concerning the patient outcome, no statistically significant difference regarding the clinical course after incorrect chest tube placement could be identified.

Management of thoracic trauma in emergency service: Analysis of 1139 cases.

Abstract
OBJECTIVE: Thoracic trauma is a common cause of significant morbidity and mortality. This study presents a series of thoracic trauma with the aim to assess epidemiologic features, distribution of pathologies, additional systemic injuries, diagnosis, management and outcome. METHODOLOGY: Between January 2007 and December 2011, all patients with thorax trauma admitted to the emergency service of our hospital were retrospectively reviewed with respect to age, gender, etiological factors, distribution of pathologies, additional systemic injuries, diagnosis, treatment modalities, referral and outcome. RESULTS: A total of 1139 patients with thorax trauma were included in the study. Of these, 698 (61.3%) were male and 441 (38.7%) were female, and the average age was 54.17±17.39 years. 1090 (95.7%) of the patients had blunt trauma, whereas 49 (4.3%) had penetrating trauma. Etiological factors were falls in 792 (69.5%), motor vehicle accidents in 259 (22.8%), animal related accidents in 39 (3.4%) and penetrating injuries in 49 (4.2%) patients. It was found that 229 (20%) patients had single, 101 (8.9%) had double, 5 (3%) had three or more, 10 (0.9%) had bilateral rib fractures and 19 (1.7%) had sternal fracture. Pneumothorax was diagnosed in 58 (5.1%) patients, whereas hemothorax, hemopneuomothorax and other systeminjuries were diagnosed in 36 (3.2%), 38(3.3%) and 292 (25.6%) respectively. In our series, thirteen patients (mortality rate 1.1%) died as result of hemorrhagic shock (n=8), respiratory distress (n=3) and severe multiple trauma (n=2). CONCLUSION: Although majority of the patients with thorax trauma receive treatment as outpatients; thoracic traumas may be a life threatening condition, and should be identified and treated immediately. Mortality varies based on etiological factors, additional systemic pathologies, capabilities of the hospital especially diagnostic and treatment facilities in emergency services. We believe that a multidisciplinary approach to the patients with severe thorax trauma, and the opportunities of emergency bedside thoracotomy in emergency services will significantly reduce the morbidity and mortality.

Los beneficios del acceso abierto al conocimiento para la investigación europea

Los resultados definitivos de RECODE, un proyecto europeo revolucionario destinado a fomentar el acceso abierto a los datos científicos en Europa, se debatirán en un congreso a celebrar en enero de 2015 en Atenas. En él se evaluará la viabilidad y los beneficios sociales y económicos potenciales que reporta el hecho de facilitar el acceso a conocimientos punteros.

El acceso abierto garantiza la disponibilidad en Internet sin trabas de investigaciones académicas revisadas inter pares. Esta estrategia eficaz y rentable puede ayudar a que Europa mantenga su excelencia científica al permitir que otros científicos consulten y amplíen descubrimientos ajenos sin restricciones. Instituciones nacionales e internacionales ya han demostrado en los últimos años un interés creciente por apoyar este tipo de distribución libre de publicaciones y datos científicos. Además, el desarrollo del acceso abierto se ha visto beneficiado por progresos tecnológicos que han dado vía libre a nuevas oportunidades de comunicación y colaboración entre científicos.

El acceso abierto permite que los contribuyentes puedan contemplar los resultados de su inversión. Al fin y al cabo, buena parte de la investigación científica y médica se financia con fondos públicos. El acceso abierto también ofrece a profesores y estudiantes la posibilidad de consultar los resultados más recientes de las investigaciones realizadas en todo el mundo. El proyectoRECODEPolicy Recommendations for Open Access to Research Data in Europe, se inició en 2013 en respuesta a la necesidad de establecer y apoyar una infraestructura de acceso abierto y de crear un foro de colaboración entre agentes europeos interesados en dar con soluciones comunes.

Gracias a su labor se han redactado recomendaciones políticas en apoyo al acceso abierto destinadas a administraciones nacionales y financiadores científicos, centros de investigación, gestores de datos y editores. Estas recomendaciones se presentarán de manera oficial en enero en Atenas.

El proyecto también logró alimentar un debate más amplio sobre el acceso abierto a los datos científicos. En Atenas se debatirán distintos temas fundamentales y buenas prácticas que permitan reutilizar datos, datos científicos abiertos y culturas científicas, datos administrativos y la evolución futura del acceso abierto. El intercambio de buenas prácticas y el empleo de los modelos ya existentes a modo de base contribuirán a reducir los costes asociados al acceso abierto.

El equipo del proyecto RECODE reconoce que la transición hacia el acceso abierto precisa de inversiones adicionales y de un cambio en la cultura científica. Para lograrlo aunó redes, comunidades y proyectos ya existentes a fin de que colaborasen en pos de soluciones comunes a retos diversos. Dichos retos comprenden los valores de las partes interesadas, aspectos jurídicos y éticos, retos tecnológicos y de infraestructura y retos institucionales.

Muchas de estas organizaciones interesadas ya trabajan para eliminar las barreras más importantes que se interponen al acceso abierto, pero su labor es aislada o comporta un contacto limitado con otras entidades. RECODEproporcionó un espacio para la colaboración entre los agentes europeos interesados en promover el acceso abierto a los datos científicos.

La organización del próximo congreso corre a cargo del Centro Nacional de Documentación de Grecia y su celebración tendrá lugar en la Fundación Nacional Helena de Investigación, en el centro de Atenas.

Abstract
BACKGROUND: Aspiration can cause a diverse spectrum of pulmonary disorders some of which can lead to death but can be difficult to diagnose. PATIENTS AND METHODS: The medical records and autopsy findings of 57 consecutive patients in whom aspiration was the immediate cause of death at Mayo Clinic (Rochester, MN, USA) over a 9-yr period, from January 1 2004 to December 31 2012 were analyzed. RESULTS: The median age at death was 72 years (range, 13-95 years) and included 39 (68%) males. The most common symptom before death was dyspnea (63%) and chest radiography revealed bilateral infiltrates in the majority (81%). Most common precipitating factors for aspiration were depressed consciousness (46%) and dysphagia (44%). Aspiration-related syndromes leading to death were aspiration pneumonia in 26 (46%), aspiration pneumonitis in 25 (44%), and large airway obstruction in 6 patients (11%). Aspiration was clinically unsuspected in 19 (33%) patients. Antimicrobial therapy had been empirically administered to most patients (90%) with aspiration pneumonia and aspiration pneumonitis. CONCLUSION: We conclude aspiration-related deaths occur most commonly in the elderly with identifiable risks and presenting bilateral pulmonary infiltrates. One-third of these aspiration-related pulmonary syndromes were clinically unsuspected at the time of death.

Identificado el mecanismo que graba el miedo en el cerebro

Bajo la sombra de los rascacielos de Manhattan, muy cerca de los 20.000 cadáveres sepultados en un viejo cementerio oculto bajo el Washington Square Park, se encuentra el laboratorio del miedo. Allí, bajo las órdenes del neurocientífico y rockero estadounidense Joseph LeDoux, trabaja una quincena de investigadores para intentar comprender por qué, por ejemplo, una persona se queda paralizada al ver a un perro, traumatizada por un huracán o muda al intentar hablar en público. Uno de los miembros de esta brigada de élite del miedo, empotrada en el Centro para la Ciencia Neural de la Universidad de Nueva York, es el neurocientífico español Lorenzo Díaz-Mataix, que acaba de identificar los mecanismos cerebrales que convierten las experiencias desagradables en recuerdos imborrables durante años.

Díaz-Mataix se ha sumergido en el cráneo de cientos de ratas. En lo más profundo de sus cerebros, como en los de los seres humanos, se esconde la amígdala, una región del tamaño de una almendra en las personas a la que la comunidad científica señala como almacén del miedo. Presuntamente, en ella se guardan durante décadas los recuerdos de las vivencias traumáticas sufridas a lo largo de la vida. Y por ella el grupo de rock de LeDoux se llama The Amygdaloids. Es el minúsculo archivo del terror en el kilo y medio de cerebro humano.

En 2010, salió a la luz el caso de una mujer estadounidense de 44 años con la amígdala completamente dañada por una rarísima enfermedad genética. La mujer, conocida como SM para preservar su anonimato, era incapaz de sentir miedo. Un grupo de investigadores encabezado por el psicólogo Justin Feinstein, de la Universidad de Iowa, siguió su pista durante más de 20 años. Rodearon a SM de serpientes y arañas venenosas, vieron con ella películas de terror como El resplandor y El silencio de los corderos, la acompañaron a sanatorios abandonados supuestamente habitados por fantasmas. Y nada. La mujer sin amígdala ni siquiera sintió miedo cuando, caminando de noche por un parque solitario, un yonqui le puso un cuchillo en la garganta y masculló: "Te voy a rajar...". SM siguió andando como escuchara La Traviata. Ahora, Díaz-Mataix ha iluminado ese enigmático cajón de recuerdos que es la amígdala cerebral. Su investigación parte de una hipótesis postulada en 1949 por el psicólogo canadiense Donald Hebb y sugerida hace más de un siglo por el nobel español Santiago Ramón y Cajal. "Dos células o sistemas de células que están repetidamente activas al mismo tiempo tenderán a convertirse en 'asociadas', de manera que la actividad de una facilitará la de la otra", dejó escrito Hebb en su libro La organización de la conducta. O, expresado de manera más simplificada, las neuronas de la amígdala del cerebro humano que se excitan eléctricamente tras el ataque de un perro permanecen conectadas durante años. Sus puentes eléctricos se refuerzan. Ese sería el esqueleto del recuerdo.

El equipo de Díaz-Mataix ha demostrado que la teoría de Hebb es cierta, al menos parcialmente, en los complejos cerebros de los mamíferos. Su experimento, cuyos resultados se publican en la revista científica PNAS,Proceedings of the National Academy of Sciences, es una versión sofisticada del célebre perro de Pávlov, aquel can ruso que se acostumbró a escuchar un metrónomo (sustituido por una campanita en el imaginario colectivo) antes de comer y ya salivaba cada vez que escuchaba el tic tac aunque no hubiera alimento. El investigador español, en tándem con Josh Johansen, del Instituto RIKEN de Ciencias del Cerebro en Japón, sometió a decenas de ratas a un pitido de 20 segundos rematado por una descarga eléctrica de medio segundo. A partir de entonces, las ratas se quedaban paralizadas cada vez que escuchaban ese sonido. En su cerebro quedó grabado el miedo al chispazo. Ahí empezó la sofisticación del experimento, gracias a una técnica conocida como optogenética. Los investigadores instalaron genes de algas sensibles a la luz a bordo de virus, que funcionan como taxis microscópicos, y los inyectaron en los cráneos de las ratas. Una vez insertados en las neuronas de los roedores, los genes eran capaces de producir una proteína que funciona como un interruptor de la célula, activándola o desactivándola en función de ráfagas de luz láser enviadas por los científicos.

Las ratas con la amígdala cerebral apagada eran incapaces de recordar el chispazo y carecían de conexiones reforzadas entre sus neuronas. Al mismo tiempo, activar las amígdalas de ratas que no habían sufrido la pequeña electrocución servía para generar miedo al pitido sin necesidad de ningún tipo de shock. En este último caso, según los autores, era necesario que se activaran también los receptores de noradrenalina, una molécula cerebral implicada en los procesos de atención. Sin esta activación, no había aprendizaje. "Con una sola descarga eléctrica asociada a un pitido, las ratas ya recuerdan la experiencia toda su vida. El cerebro hace esto para afrontar los peligros. Un animal necesita aprender con una sola oportunidad, porque quizá no tenga otra", explica el neurocientífico.

El despacho del también español Luis de Lecea, profesor de Psiquiatría en la Universidad de Stanford (EE.UU.), se encuentra a escasos 15 metros del laboratorio en el que se desarrolló la optogenética en 2004. Desde allí, De Lecea ha sido testigo de cómo esta técnica ha revolucionado la investigación del cerebro humano. Las teorías de Hebb ya se habían prácticamente confirmado "con rodajas de cerebro" de roedores en el laboratorio, pero los experimentos de Díaz-Mataix son "una demostración elegante" en mamíferos vivos, a juicio de De Lecea. El neurocientífico español dibuja las posibles aplicaciones de sus hallazgos. "En los enfermos con estrés postraumático, ansiedad o incluso depresión, su cerebro no es capaz de aprender que lo que una vez fue peligroso ya no lo es, y siguen respondiendo de forma exagerada", señala. Personas que han vivido guerras, accidentes graves, violaciones o catástrofes naturales siguen sintiendo miedo y estrés una vez pasado el peligro.

La comunidad científica internacional trabaja desde hace unos años en intentar borrar esos malos recuerdos. Se basan en un proceso conocido como reconsolidación de la memoria. "Cada vez que un recuerdo sale a la luz, se pone en un estado frágil que hace que el cerebro pueda añadir cosas relevantes", apunta Díaz-Mataix. Cuando se abre el baúl de los recuerdos es el momento de modificarlos. Si, por ejemplo, alguien va en un coche escuchando a todo volumen la canción Balada Boa de Gusttavo Lima y se estampa contra un árbol, cada vez que escuche el estribillo 'Tchê tcherere tchê tchê' tendrá pavor. "Sin embargo, si cada vez que la víctima va a un bar a tomar algo ponen esa canción, el cerebro recupera el recuerdo y aprende que ya no es negativa. Eso es la reconsolidación", añade el investigador. Este proceso se puede facilitar con fármacos que actúan sobre los receptores de noradrenalina, como el propranolol, que ya se suministró a víctimas del atentado del 11 de marzo de 2004 en Madrid. Los síntomas de su trastorno de estrés agudo remitieron en el 64% de los casos, según un estudio de la mutua Ibermutuamur.

Para Díaz-Mataix, es muy posible que el proceso para almacenar recuerdos desagradables que han observado sea en realidad un mecanismo general del sistema nervioso para generar otro tipo de recuerdos, ya sean de asco, ira o alegría. "El problema es cómo estudiar estas emociones primarias en una rata", lamenta. Si tiene razón, será todavía más cierta aquella sentencia de Ramón y Cajal: "Todo hombre puede ser, si se lo propone, escultor de su propio cerebro".

Abstract
Background: In order to alleviate pain, reduce scarring, and to improve the overall outcome in burn patients multitude of novel agents are being utilized. In this regard, heparin has been introduced because of its research proven role in burn wound management. Objective:The objective was to evaluate, whether the addition of heparin, administered only topically, could improve burn treatment. Materials and Methods: The subjects in this study were 40 consecutive burn patients with 10-20% burns, randomly allocated to heparin group (H-group) (20) and control group (C-group) (20). Results: All patients in the H-group were receiving analgesics on demand only from the 2 nd week onwards. This was in contrast to the C-group wherein 35% patients received twice daily dosage of analgesics in the 2 nd week of their treatment. When compared to the average hospital stay of 18.3 days in the C-group, patients belonging to the H-group had an average hospital stay of 12.3 days (P < 0.05). Conclusions: The current comparative study demonstrates that heparin significantly decreases the requirement of analgesics and the time required to prepare a burn wound for grafting. Besides as compared to silver sulfadiazine dressings, heparin appears to be cost-effective.

Postburn pruritus: A practical review.

Abstract
The incidence of postburn pruritus is reported to vary between 80% and 100% and the persistence of itching leads to disabling symptoms such as sleep disturbance, anxiety, and disruption of daily activities. Recently, a few small randomized controlled trials by investigators have focused our attention to the neurobiology and molecular mechanisms of the postburn pruritus and the role of centrally acting agents in its treatment. It is now recognized that the central nervous system develops aberrant autonomous activity that causes maintenance of pruritic symptoms into a chronic state. This practical review on the topic aims to rationalize and simplify the current treatment options, through emerging and available evidence, to enable the physician to make an even better informed choice. While antihistamines and massage therapy will continue to be effective first-line strategy for most clinicians, the promising results in controlled studies obtained with gabapentin/pregabalin to ameliorate pruritic symptoms in a predictable fashion in burns patients have caused a paradigm shift in the therapeutic approach. Tailoring the drug therapy to the severity of symptoms leads to more successful therapy of this vexing malady and current evidence supports the use of gabapentin/pregabalin in patients with moderate to severe postburn pruritus (visual analog scale score >5) as the first line, even if as an "off label" indication. Other treatment modalities like doxepin, ondansetron, or transcutaneous electrical nerve stimulation may have applications on a case to case basis. However, massage therapy should continue to be an adjunct with all other modalities.
