Abstract

BACKGROUND:

Developmental dysplasia of the hip (DDH), which encompasses a wide spectrum of disease from mild dysplasia to frank dislocation, is one of the most common developmental deformities of the lower extremities and one of the leading causes of future osteoarthritis and hip arthroplasty. Legg-Calvé-Perthes disease (LCPD) results from a vascular insult to the growing femoral epiphysis, which in turn can create permanent morphologic changes to the hip joint. Slipped capital femoral epiphysis (SCFE) occurs when the proximal femoral physis fails allowing the epiphysis to displace in relation to the metaphysis. Infections about the hip also create significant morbidity in the pediatric hip.

CONCLUSIONS:

DDH, LCPD, SCFE, and infections about the hip continue to be important topics in pediatric orthopaedics and areas of vital research. This manuscript reviews the most important recent literature on the diagnosis and treatment of these pediatric hip conditions.


Resumen

ANTECEDENTES:

La displasia del desarrollo de la cadera (DDC), que abarca un amplio espectro de enfermedad, desde displasia leve a luxación franca, es una de las deformidades de desarrollo más comunes de las extremidades inferiores y una de las principales causas de osteoartritis y artroplastia de cadera en el futuro. La enfermedad de Legg-Calvé-Perthes (LCPD) es el resultado de una lesión vascular en la epífisis femoral en crecimiento, que a su vez puede crear cambios morfológicos permanentes en la articulación de la cadera. La epífisis femoral mayúscula deslustrada (SCFE) ocurre cuando la fisis femoral proximal falla y permite que la epífisis se desplace en relación con la metáfisis. Las infecciones sobre la cadera también crean una morbilidad significativa en la cadera pediátrica.

CONCLUSIONES:

DDH, LCPD, SCFE e infecciones sobre la cadera continúan siendo temas importantes en ortopedia pediátrica y áreas de investigación vital. Este manuscrito revisa la literatura reciente más importante sobre el diagnóstico y tratamiento de estas afecciones pediátricas de cadera.
