[Identidad Bibliotecaria] La red también lee

Aunque los medios de difusión principales de su campaña tal vez no sean los más eficientes (yo diría que el público al que necesitan llegar no es, precisamente, el que se mueve en portales y medios online de literatura, lectura, y cultura en general), estoy bastante de acuerdo con muchos de los aspectos que exponen los promotores de La red también lee. Si bien ahora mismo están predicando en terreno ganado, esperemos que sus ideas lleguen también a otros medios, más tradicionales, para que su voz se escuche más allá de la aglomeración en línea.

¿Y qué es lo que reivindican, exactamente? En su manifiesto o carta abierta, concluyen exponiendo: Los firmantes de esta carta pedimos que, a la hora de reconocer labores relacionadas con la promoción cultural, Internet sea considerado un medio como los otros, ni mejor ni peor, simplemente más interactivo/participativo. Y que el valor de los contenidos que aquí vertimos sea evaluado con el mismo interés y el mismo rigor que los de cualquier otro medio. En definitiva, que los organismos e instituciones del mundo de la lectura y la edición nos juzguen por lo que hacemos, no por dónde lo hacemos.

Perdidos como estamos en estos mundos virtuales de foros, webs, blogs, redes y libro digital, se nos olvida a veces que ahí fuera hay un mundo que puede ser muy diferente: organismos que se agarran a modelos conservadores de fomento cultural; editoriales que se olvidan de que el servicio de prensa va más allá de un par de revistas especializadas en papel; profesores y padres que ignoran la inmensa oferta literaria que espera a sus alumnos e hijos más allá de los libros de texto… Y esto es lo que pretenden recordarnos los creadores de la propuesta, Eva Orúe (Divertinajes, Lucía Bartolomé (¡¡Ábrete, libro!!), Carmen Fernández Etreros (Pizca de papel) y Emilio Ruiz Mateo (Estandarte), que insisten en que la literatura también se vive en Internet. Para la difusión de su carta abierta ofrecen algunas ideas: proponen enviarla por email a todos los que podrían estar implicados (amigos, webs y blogs de literatura, medios de comunicación y, por supuesto, la Federación de Gremios de Editores de España en prensa@fge.es); ponerla en nuestro muro de Facebook; enviarla por Twitter con el hashtag #laredtambienlee; y colocarla en nuestras webs o blogs. Consideran que la Federación de Gremios de Editores en particular necesita de un buen recordatorio de la relevancia de internet en el mundo de lo literario (al respecto, podéis leer el artículo con el que comenzó toda esta iniciativa).
Una de las recriminaciones más frecuentes que se lanza al mundo editorial español es la lentitud de su adaptación al nuevo mercado electrónico y a las nuevas tecnologías en general, por lo que lectores y entusiastas de la literatura tienen ahora una oportunidad más para reivindicar el importantísimo papel de la world wide web en el mundo de la crítica, la opinión, el intercambio de ideas, la promoción y todas las demás esferas del mundo editorial, incluyendo, cómo no, la mismísima lectura.

[Identidad Bibliotecaria] Futuras ferias sin libros de papel?

Como casi cada año, acabo de darme una vuelta a la Feria Internacional del Libro de Monterrey, gran evento en Cintermex que atrae muchos miles de lectores ávidos de conocer las novedades editoriales y que termina este próximo domingo.

La edición de este año tiene dos novedades: en primer lugar, está dedicada al fallecido escritor Carlos Fuentes (puede uno ver que las "calles" tienen nombres de libros escritos por el ilustre autor). En segundo lugar, esta vez los organizadores (Tec de Monterrey) abrieron un espacio para los libros electrónicos o "e-Books", para mostrar una apertura hacia las nuevas tendencias tecnológicas en materia de lectura.

El área de e-Books está situada en el pasillo principal, no en el área dedicada a los libros "propiamente dichos". En mi opinión, esto muestra, por una parte, que los e-Books todavía no son considerados como libros por derecho propio, pero por otra parte tuvo la ventaja de la mayor visibilidad, pues los e-Books es lo primero que encuentra el visitante a su paso.

En el área de eBooks encontramos como stand principal el de Amazon, quienes muestran en computadoras el portal de libros en español. En efecto, ahora Amazon tiene un portal especialmente para el público hispanohablante, en la dirección http://www.amazon.com/tiendakindle, el cual, aunque en principio está destinado a la comunidad hispana de Estados Unidos, nos sirve perfectamente a los mexicanos que tenemos tarjeta de crédito. Me llamó la atención que los dos primeros lugares de ventas en la tienda Kindle son dos novelas eróticas, "Desnuda ante ti" y "Cincuenta sombras de Grey", lo cual aparentemente indica que los latinos somos cachondos… y también que es mucho más privado leer un libro electrónico que uno de papel, pues éste último nos delata por su portada.

Estuve además en dos conferencias sobre e-Books; en una de ellas, Jose Alberto Parra, responsable del negocio Kindle Amazon para México y Colombia nos presentó el antes mencionado portal de Kindle en español, y también impresionantes estadísticas que muestran el enorme crecimiento de los libros electrónicos. Una de las cosas que mencionó José es que las personas que adquieren un lector Kindle, en los años siguientes leen 3 veces más que antes. Por lo pronto en Amazon la venta de libros Kindle ya superó en cantidad la de libros en papel, y esta tendencia se acentúa día con día.

La evolución hacia el contenido digital es irreversible. Y si no me creen, juzguen por ustedes mismos: esta semana la revista americana "Newsweek" anunció que este año se termina su edición en papel, y a partir de 2013 (si el mundo sobrevive y las profecías son falsas) sólo se distribuirá en suscripción digital. Otro dato: en música, las ventas digitales sobrepasaron a las de soporte físico -y estoy diciendo "ventas", lo que excluye la piratería electrónica.

En días pasados hablé con el director del comité organizador de la Feria del Libro, Miguel A. Arreola, también director de la biblioteca del Tec de Monterrey, y le preguntaba que cómo se imaginaba que podrían ser las ferias del libro del futuro, si efectivamente los libros en papel desaparecen. ¿Qué van a hojear los aficionados a la letra impresa? Me comentó Miguel Angel que en su opinión falta mucho para eso, pero que aún sin libros físicos puede haber ferias de libros, que estarían compuestas de eventos de presentación de libros (las cuales ya hoy son una de las principales atracciones), y habría muchas pantallas desplegando las novedades, así como códigos QR de esos que leen los celulares con la cámara.

Yo no podría estar más de acuerdo con Miguel Angel Arreola: lo esencial de la lectura no es el olor del papel, sino un contenido de calidad. Una buena novela (como "Un día perfecto" de Ira Levin, que actualmente estoy leyendo), será buena en papel o en lector electrónico.
Visiten la Feria del Libro en Cintermex hoy, mañana o el domingo, y no dejen de ir cuando los libros de papel sean reliquias de museo.

Abstract
Pulmonary complications are a major cause of morbidity and mortality in neurosurgical patients. The common pulmonary complications in neurosurgical patients include pneumonia, postoperative atelectasis, respiratory failure, pulmonary embolism, and neurogenic pulmonary edema. Postoperative lung expansion strategies have been shown to be useful in prevention of the postoperative complications in surgical patients. Low tidal volume ventilation should be used in patients who develop acute respiratory distress syndrome. An antibiotic use policy should be put in practice depending on the local patterns of antimicrobial resistance in the hospital. Thromboprophylactic strategies should be used in nonambulatory patients. Meticulous attention should be paid to infection control with a special emphasis on hand-washing practices. Prevention and timely management of these complications can help to decrease the morbidity and mortality associated with pulmonary complications.

Abstract
BACKGROUND:
Post-operative pulmonary infection often appears to result from aspiration of pathogens colonizing the oral cavity. It was hypothesized that impaired periodontal status and pathogenic oral bacteria significantly contribute to development of aspiration pneumonia following neurosurgical operations. Further, the prophylactic effects of a single dose preoperative cefazolin on the oral bacteria were investigated. METHODS: A matched cohort of 18 patients without postoperative lung complications was compared to 5 patients who developed pneumonia within 48 hours after brain surgery. Patients waiting for elective operation of a single brain tumor underwent dental examination and saliva collection before surgery. Bacteria from saliva cultures were isolated and periodontal disease was scored according to type and severity. Patients received 15 mg/kg cefazolin intravenously at the beginning of surgery. Serum, saliva and bronchial secretion were collected promptly after the operation. The minimal inhibitory concentrations of cefazolin regarding the isolated bacteria were determined. The actual antibiotic concentrations in serum, saliva and bronchial secretion were measured by capillary electrophoresis upon completion of surgery. Bacteria were isolated again from the sputum of postoperative pneumonia patients. RESULTS: The number and severity of coexisting periodontal diseases were significantly greater in patients with postoperative pneumonia in comparison to the control group (p = 0.031 and p = 0.002, respectively). The relative risk of developing postoperative pneumonia in high periodontal score patients was 3.5 greater than in patients who had low periodontal score (p < 0.0001). Cefazolin concentration in saliva and bronchial secretion remained below detectable levels in every patient. CONCLUSION: Presence of multiple periodontal diseases and pathogenic bacteria in the saliva are important predisposing factors of postoperative aspiration pneumonia in patients after brain surgery. The low penetration rate of cefazolin into the saliva indicates that its prophylactic administration may not be sufficient to prevent postoperative aspiration pneumonia. Our study suggests that dental examination may be warranted in order to identify patients at high risk of developing postoperative respiratory infections

Abstract
Deep venous thrombosis (DVT) and pulmonary embolism (PE) cause significant morbidity and mortality in orthopaedic surgical practice, although the incidence following surgery to the lumbosacral spine is less than following lower limb surgery. Our objective was to compare our rate of thromboembolic complications with those published elsewhere and investigate whether the adoption of additional pharmacological measures reduced the incidence of clinically evident DVT and PE. This retrospective study was undertaken to investigate the incidence of DVT/PE during the 10 years from 1 January 1985 to 31 December 1994, and then to assess the effectiveness of an anticoagulant policy introduced during 1995 using low dose aspirin or LMH in high risk cases. All records for spinal operations were reviewed for thrombo-embolic complications by reference to the Scottish Morbidity Record form SMR1. To ensure that all patients were compliant with the policy, data for the whole of 1995 was omitted and the period 1 January 1996 to 31 December 2003 was taken to assess its effectiveness. Surgery was done with the patient in the kneeling, seated prone position which leaves the abdomen free and avoids venous kinking in the legs. Records of a total of 1,111 lumbar spine operations were performed from 1 January 1985 to 31 December 2004 were reviewed. The overall incidence of thrombo-embolic complications was 0.29%. A total of 697 operations were performed from 1 January 1985 to 31 December 1994 with two cases of DVT and no cases of PE giving thromboembolic complication rate of 0.29%. During the period 1 January 1996 to 31 December 2003, 414 operations resulted in one case of DVT and no cases of PE, a rate of 0.24%. The incidence of symptomatic thrombo-embolic complications in lumbar spinal surgery is low in the kneeling, seated prone operating position, whether or not anticoagulation is used

Abstract
Post-herpetic neuralgia is a neuropathic pain syndrome resulting from an insult to the peripheral and central nervous systems caused by the varicella zoster virus. Spontaneous pain may result in the persistent sensation of burning, tingling, or aching and may be associated with thermally or mechanically provoked pain, resulting in hyperalgesia or allodynia. The majority of cases occur in patients over the age of 50 years. Gabapentin is a structural analog of gamma aminobutyric acid that binds to the α(2)-δ site of voltage-dependent calcium channels and modulates the influx of calcium, with a resulting reduction in excitatory neurotransmitter release. Gabapentin is effective in reducing neuropathic pain due to post-herpetic neuralgia when given at least three times per day, due to its short half-life, resulting in demonstrable fluctuations in plasma levels. Gabapentin has dose-limiting side effects that prevent some patients from achieving therapeutic plasma levels, such as somnolence (27.4%), dizziness (23.9%), and ataxia (7.1%). Gralise™ is a once-daily extended-release formulation of gabapentin that has been developed using AcuForm™ technology. AcuForm is a polymer-based drug delivery system that retains the tablet in the stomach and upper gastrointestinal tract for a sustained period of time. Once-daily dosing has been shown to provide comparable drug exposure with an identical daily dose of the immediate-release formulation when administered three times daily. Participants given Gralise 1800 mg daily had a statistically significant reduction in average daily pain intensity scores compared with placebo, reduced sleep interference due to pain, and a greater percent of participants reporting being much or very much improved on the patient global impression of change. An analysis comparing the efficacy and safety profiles in the aging population (≥65 years) with those younger than 65 years showed that Gralise is effective and well tolerated in both age groups.

Abstract
OBJECTIVE: An immediate-release formulation of gabapentin is approved for treatment of postherpetic neuralgia (PHN). This formulation, however, requires multiple daily dosing, usually three times per day, and is associated with a high incidence of somnolence and dizziness. We assessed the tolerability and safety of a once-daily gastroretentive formulation of gabapentin (G-GR) in phase 3 clinical trials in patients with PHN.RESEARCH DESIGN AND METHODS: Data were pooled from two placebo-controlled studies involving 723 patients (G-GR 1800 mg, n = 359; placebo, n = 364). Patients (43% male, mean age 66 years) with PHN pain >4 (0-10 scale) for ≥3 months were enrolled. Summary statistics for the incidence of treatment-emergent adverse events (AEs) were performed. Laboratory parameters and vital signs were assessed. RESULTS:
Treatment-emergent AEs were reported in 48% of patients (G-GR, 54%; placebo, 42%) and led to discontinuation in 8% of patients (G-GR, 10%; placebo, 7%). The most frequent (≥3% in any group) AEs were dizziness (G-GR, 11%; placebo, 2%), somnolence (G-GR, 5%; placebo, 3%), headache (G-GR, 4%; placebo, 4%), peripheral edema (G-GR, 4%; placebo, <1%), and diarrhea (G-GR, 3%; placebo, 3%). Serious AEs were reported in seven patients in the G-GR group (2%) and ten patients in the placebo group (3%). There were two deaths, both in the placebo group. No serious AEs were considered related to treatment. Mean values for laboratory parameters and vital signs at the end of each study were similar between groups. CONCLUSION: G-GR was safe and well tolerated for the treatment of PHN.

Abstract
Postherpetic neuralgia (PHN) is a devastating, chronic pain syndrome that can develop following an outbreak of herpes zoster and becomes increasingly common as patients age. PHN can be difficult to treat and often requires trials of multiple agents to achieve significant pain relief. Pregabalin is the newest agent to gain approval for PHN. Data suggest efficacy for relief of pain and sleep disturbance secondary to PHN in affected patients. Although there are no head-to-head comparisons, pregabalin appears comparable to gabapentin and other first-line agents for treating PHN.
