Anticoncepción y adolescencia, ¿lo tenemos bien resuelto?

A pesar de que en la página "Acerca del blog" comento que mi idea no es responder consultas médicas, lo cierto es que me llegan muchísimas consultas y comentarios a través del correo del blog. Muchas veces son comentarios que me motivan y que me dan ideas para nuevas historias. Otras veces son consultas generalizables para las que se puede dar un link o redirigir a la persona a una solución a su problema. Intento ayudar en lo posible, pero no siempre puedo ni me apetece. Además, en ciertos casos, las historias son tan concretas y me piden tal grado de implicación que no puedo asumir.


De todos estos correos hay algunos que me llegan al alma. Gente que se desnuda dejando confesiones y experiencias duras y gente que me dice cosas muy bonitas y emocionantes.


Hace unas cuantas semanas, recibí un correo de una chica de 17 años a la que llamaré Marta. No sé cómo llegó a estas páginas, pero me imagino que navegando en internet intentando dar solución a sus preocupaciones. El caso es que me relataba una historia de relaciones sexuales de riesgo y me planteaba cómo solucionar sus miedos a un posible embarazo y cómo gestionar el acceso a los anticonceptivos. Tenía miedo a consultar a los profesionales sanitarios por temor a que sus padres acabaran sabiéndolo. Su correo era una muestra de todos los tópicos -que no por ello dejan de ser la realidad de la calle-: chicos y chicas jóvenes iniciándose en las relaciones sin una información correcta de hábitos saludables, conocimientos de anticoncepción y formas de evitar las enfermedades de transmisión sexual, desconocimiento del sistema sanitario y de los conceptos de autonomía del paciente y de confidencialidad, irresponsabilidad en algunos casos del sector "masculino", tema tabú a nivel familiar, etc.





No hace falta que os explique que contesté a ese correo en un tiempo récord y unos minutos antes de apagar las luces e irme a dormir. Aunque estaba reventada, no podía dejar a esa chica colgada con sus angustias y sin los recursos que tenemos los "adultos" para afrontar los problemas.


Le informé someramente sobre las posibilidades de anticoncepción, pero sobretodo le comenté que si tenía dudas tenía que consultar sin miedo. Que los profesionales sanitarios tenemos obligación de mantener la confidencialidad, y que a partir de los 16 años, el menor es en realidad mayor de edad para lo sanitario, y además me atreví a darle un par de consejos. Primero que hablara con su pareja. La anticoncepción no es un asunto que se deba dejar para el momento "D". Hay que tenerlo hablado, consensuado y aprender juntos si es que no se sabe demasiado. El segundo consejo: que intentara hablar con sus padres, o como mínimo con su madre. A veces los padres pueden parecer conservadores y tener la sensación de que pueden resultar intransigentes y castigadores con algunos temas y sin embargo les sacan las castañas del fuego a sus jóvenes hijos (y no tan jóvenes) cuando estos se meten en líos.


Le pedí a Marta, al final de mi correo, que por favor me explicara cómo había acabado la historia. La situación me había provocado una intensa desazón. Me escribió de nuevo hace unos días. Parece que pudo solucionar su preocupación de ese momento de forma satisfactoria. Además pudo hablar con su pareja e incluso se planteó poder sincerarse con su madre. Así que podríamos decir que algo hemos avanzado. Sin embargo me pregunto cuántos chicos y chicas pasan por circunstancias similares en soledad.


Toda esta historia me recuerda a un artículo que leí en El País a principios de mes titulado"¿Anticonceptivos en el instituto? Funciona" . En dicho reportaje partía de un programa de planificación familiar que se está desarrollando en el Reino Unido en varios centros escolares. En dicho programa se facilitaba el acceso a la anticoncepción a jóvenes (muy jóvenes) sin el consentimiento familiar. La polémica está servida. Pero en lo que sí parecen ponerse de acuerdo todos los expertos es en que poner barreras al acceso a la anticoncepción (sociales, económicas, por edad) no hace que los jóvenes dejen de mantener relaciones, y éstas se producen asumiendo muchos más riesgos.

ILUMINACIÓN DE MANUSCRITOS

Ya se edito un post al respecto en octubre del 2008 odisea2008: EL ARTE DE LA ILUMINACIÓN DE MANUSCRITOS , he encontrado sin embargo dos libritos interesantes sobre el tema que pueden servir de complemento al post anterior. Y son, el primero Lessons in the art of illuminating. A series of examples selected from works in the British Museum, Lambeth Palace Library, and the South Kensington Museum (lecciones del arte de la iluminación. Una serie de ejemplos seleccionados de obras en el British Museum, Biblioteca Lambeth Palace y el South Kensigton Museum), ENLACEeditado en 1880 y escrito por William John Loftie, nos presenta un esquema general de este arte acompañado de laminas en blanco y negro y color sobre iluminaciones diversas, explicadas en detalle.

Veamos unos ejemplos, para ampliar pique sobre la imagen y una vez en el servidor escoja tamaño original.

El siguiente libro es A practical treatise on the art of illuminating : with examples, chromographed in fac-simile and in outline, of the styles prevailing at different periods, from the sixth century to the present time ENLACE (Tratado practico del arte de la iluminación: con ejemplos, cromatografiados en facsímil y en esbozos, de los estilos imperantes en diferentes periodos, desde el siglo sexto al presente), edición de 1873 escrita por Marcus Ward, donde al igual que el anterior y tal como refleja el titulo se estudian varios ejemplos de iluminaciones de los periodos indicados. Abstract
Pain is among the worst possible experiences for the critically ill. Therefore, nearly all intensive care patients receive some kind of pain relief, and opioids are most frequently administered. Morphine has a number of important adverse effects, including histamine release, pruritus, constipation, and, in particular, accumulation of morphine-6-glucuronide in patients with renal impairment. Hence, it is not an ideal analgesic for use in critically ill patients. Although the synthetic opioids fentanyl, alfentanil, and sufentanil have better profiles, they undergo hepatic metabolism and their continuous infusion also leads to accumulation and prolonged drug effects. Various attempts have been made to limit these adverse effects, including daily interruption of infusion of sedatives and analgesics, intermittent bolus injections rather than continuous infusions, and selection of a ventilatory support pattern that allows more spontaneous ventilation. However, these techniques at best only limit the effects of drug accumulation, but they do not solve the problem. Another type of approach is to use remifentanil in critically ill patients. Remifentanil is metabolized by unspecific blood and tissue esterases and undergoes rapid metabolism, independent of the duration of infusion or any organ insufficiency. There are data indicating that remifentanil can be used for analgesia and sedation in all kinds of adult intensive care unit patients, and that its use will result in rapid and predictable offset of effect. This may permit both a significant reduction in weaning and extubation times, and clear differentiation between over-sedation and brain dysfunction. This article provides an overview of the use of short-acting opioids in the intensive care unit, with special emphasis on remifentanil. It summarizes the currently available study data regarding remifentanil and provides recommendations for clinical use of this agent.

Remifentanil is an ultrashort-acting synthetic opioid. It is metabolized by nonspecific tissue and plasma esterases. Remifentanil's metabolism is responsible for its unique pharmacokinetic profile and flat, context-sensitive half-time. Since its introduction into clinical practice, remifentanil has been used for a variety of anesthetic and analgesic applications; however, concerns regarding a potential for rapid induction of tolerance and/or induced hyperalgesia, coupled with an ultrarapid offset of effect, make the drug less than optimal for use in the pharmacologic management of pain.

Abstract:


Infectious skin diseases pose considerable treatment challenges, especially given the recent appearance of several highly virulent pathogens as well as the rising number of immunocompromised patients in the United States. This article discusses common bacterial, fungal, and viral skin infections with an emphasis on cellulitis, dermatophyte infections, and herpes simplex viral infections. Disease pathogenesis, treatment, and cost of treatment are addressed with an emphasis on therapeutic needs and future research directions. Common priorities for all infectious skin disease categories include increased disease surveillance, study of existing treatments, and efforts in drug development.

Abstract:
Acne vulgaris is a common skin condition with substantial cutaneous and psychologic disease burden. Studies suggest that the emotional impact of acne is comparable to that experienced by patients with systemic diseases, like diabetes and epilepsy. In conjunction with the considerable personal burden experienced by patients with acne, acne vulgaris also accounts for substantial societal and health care burden. The pathogenesis and existing treatment strategies for acne are complex. This article discusses the epidemiology, pathogenesis, and treatment of acne vulgaris. The burden of disease in the United States and future directions in the management of acne are also addressed.

Psoriasis, a chronic inflammatory skin condition, is a complex disease in terms of its significant comorbidities and impact on patients' quality of life. The objective of this article is to elucidate the health outcomes of the disease, including its economic and psychosocial burden on the patient. Current treatments options and the economic considerations of treatment costs are reviewed. Psoriasis is a multidimensional disease, so patients benefit from having a multidisciplinary team of dermatologists and other physicians for management of it and of associated comorbid conditions.

Abstract
Maxillofacial trauma is without doubt still a very challenging area, especially in the early stages of care when other injuries may be present. Craniofacial trauma, with or without life and/or sight-threatening complications, may be associated with significant injuries elsewhere. Both general trauma and facial trauma management have evolved considerably over the last 20 years and on occasion clinical priorities may seemingly conflict, suddenly change or be hidden. In these circumstances a number of clinical dilemmas may arise, which this and three further reviews will discuss. These are based on review of the current literature, supplemented where appropriate by the collective experiences of the co-authors. Facial injuries can be broadly placed into one of four groups, which can aid determination of the urgency for treatment. Advanced Trauma Life Support is generally regarded as the gold standard and is founded on a number of well known principles, but strict adherence to protocols may have its drawbacks when facial trauma co-exists. These can arise in the presence of either major or minor facial injuries, and oral and maxillofacial surgeons need to be aware of the potential problems.

Abstract
Maxillofacial trauma poses an obvious threat to the patient's airway, which may not be immediately evident. In the multiply injured patient, the co-existence of actual or potential injuries elsewhere may complicate airway management, notably in the presence of full spinal immobilization. Following high-velocity trauma, injuries to the cervical spine must be assumed to be present. They also need to be ruled out in an appropriate and timely manner, as patients may wish to sit up. Assessment and management of the airway in maxillofacial trauma can be difficult, requiring a senior anaesthetist or other individual appropriately trained in emergency airway care. A number of management options may exist to protect the airway, each with advantages and drawbacks. Agitation and vomiting can occur unexpectedly and need to be managed safely with due consideration to the spine. Oral and maxillofacial surgeons need to be aware of these dilemmas and their early warning signs, and be skilled in emergency surgical airway procedures, especially if involved as part of the trauma team. Prolonged immobilization is associated with significant morbidity and mortality. A number of protocols currently exist for 'clearing' the spine. Imaging now plays a greater role, especially in the obtunded, unconscious or intubated patient, and this is discussed.

El hospital General de Castellón, a través de su Servicio de Medicina Física y Rehabilitación, celebró ayer una jornada con el objetivo de actualizar los conocimientos de los especialistas del centro en Rehabilitación Intervencionista. Un curso eminentemente práctico que contó con una serie de casos, algunos presentados en vídeo y otros en una consulta habilitada para el efecto.
Así, en el escenario del Salón de Actos del centro sanitario castellonense, la organización de este jornada instructiva dispuso de equipos de electromedicina para que el estudio de los pacientes se pudiera llevar a cabo en directo.
Tras ello, los médicos y expertos en la materia presentes pudieron discutir y evaluar las distintas posibilidades diagnósticas y el tratamiento que se derivaría de ellas en el ámbito de la Rehabilitación Intervencionista, el tema principal de este curso.
Cabe resaltar que esta novena jornada formativa del Servicio de Medicina Física y Rehabilitación del hospital General contó con la presencia de los especialistas de la Comunitat, como José María Climent, del hospital General de Alicante, y Pedro Fenollosa, del hospital La Fe de Valencia, además de los propios profesionales del centro sanitario castellonense.

Abstract
BACKGROUND AND OBJECTIVE: Our objective was to report on the design and essentials of the Etoricoxib protocol- Preemptive and Postoperative Analgesia (EPPA) Trial, investigating whether preemptive analgesia with cox-2 inhibitors is more efficacious than placebo in patients who receive either laparotomy or thoracotomy. DESIGN AND METHODS: The study is a 2 x 2 factorial armed, double blinded, bicentric, randomised placebo-controlled trial comparing (a) etoricoxib and (b) placebo in a pre- and postoperative setting. The total observation period is 6 months. According to a power analysis, 120 patients scheduled for abdominal or thoracic surgery will randomly be allocated to either the preemptive or the postoperative treatment group. These two groups are each divided into two arms. Preemptive group patients receive etoricoxib prior to surgery and either etoricoxib again or placebo postoperatively. Postoperative group patients receive placebo prior to surgery and either placebo again or etoricoxib after surgery (2 x 2 factorial study design). The Main Outcome Measure is the cumulative use of morphine within the first 48 hours after surgery (measured by patient controlled analgesia PCA). Secondary outcome parameters include a broad range of tests including sensoric perception and genetic polymorphisms. DISCUSSION: The results of this study will provide information on the analgesic effectiveness of etoricoxib in preemptive analgesia and will give hints on possible preventive effects of persistent pain.

Abstract
Background: Etoricoxib, a selective Cox-2 inhibitor has been found to be effective in the management of acute pain. This study evaluates the effect of preoperative use of oral Etoricoxib on post operative pain relief and sleep in patients undergoing single level diskectomy. Methods: In this prospective, randomized, controlled study, forty four patient (ASA 1&2,age 18-60 years) scheduled to undergo single level lumber diskectomy were given either placebo (control group) or Etoricoxib 120 mg orally one hour before surgery. Post operatively fentanyl intravenous (IV) PCA pump was started. Visual analog score (VAS ) was assessed at 0, 6, 12, 18 and 24 hours at rest and movement. Primary end point was total pain relief over 24 hours. Sleep overnight, total fentanyl consumption, incidence of nausea and vomiting, intra-operative blood loss and patient satisfaction were noted. Results: Forty three patients completed the study. Reductions in VAS at rest and on movement were observed in the Etoricoxib group when compared with the Control group at all the intervals till 24 hours postoperatively, except on movement at 24 hours postoperative (P<0.05). Total fentanyl consumption (μg/kg/hr) was higher in Control group (P=0.007). More patients in Etoricoxib group had a contented facial expression (p=0.003), relaxed body language (p=0.00) and better sleep at night than control group (p=0.0004). Conclusion: Single preoperative oral dose (120 mg) of Etoricoxib, given one hour before surgery, has significantly reduced the post operative pain at rest and movement and improved sleep in patients undergoing single level diskectomy without any side effects and with good patient satisfaction.

Abstract
OBJECTIVE: Administering cyclooxygenase-2 inhibitors preoperatively appears attractive since these drugs reduce post-operative pain, but do not increase the risk of post-operative bleeds, asthmatic attacks and stress-related gastrointestinal ulcers. In a former investigation, we could show that post-operative administration of etoricoxib reduces prostaglandin production in wound fluid, but the onset of action is variable due to delayed post-operative absorption. METHODS: In this study, we investigated the preoperative administration of etoricoxib in patients undergoing hip replacement. They received 120 mg etoricoxib or placebo 2 h before surgery and 1 day after in a double-blinded, randomized, parallel group design. RESULTS: A total of 11 patients were randomized (placebo n = 5; verum n = 6). We found high and constant levels of the drug in blood, central nervous system and wound fluid already at the end of surgery (t(max) < 2 h). This was accompanied by inhibition of prostaglandin production in the wound tissue (treatment p < 0.05), suppression of interleukin 6 increase in plasma (treatment p < 0.01), and - despite existing standard pain relief procedures - higher satisfaction with analgesics (time vs. treatment p < 0.05) and less demand for opioids (treatment p < 0.01) and intrathecal bupivacaine (treatment p = 0.05) administration. CONCLUSION: Administration of etoricoxib 2 h before surgery allows for an effective drug concentration in critical tissues, a reduction of the production of pro-inflammatory mediators and for better pain relief.

Elise Tarvainen es una experta en Educación de la Universidad de Jyväskylä, en Filandia. Este martes ha dado una conferencia, o mejor, ha impartido una clase en el WISE 2012, World Innovation Summit for Education (cumbre mundial de la innovación para la Educación), sobre “El profesor, el catalizador más importante para el éxito y la innovación en la Educación”.

La sala donde esta espigada señora nórdica ha explicado el porqué de la excelencia de la Educación en su país estaba llena. Finlandia continúa de moda.

Desde luego, ha advertido que las recetas no son exportables al cien por cien, entre otras razones porque la educación es algo que está muy enraizado en la propia cultura, historia y religión. Pero sí se ha atrevido a dar unos principios generales.

El objetivo fundamental que un profesor se ha de proponer en su aula, afirma, es “excitar el deseo de aprender” y “mejorar el flujo del aprendizaje”. Esto segundo se traduce en no permitir que nadie quede parado en el aula, tímido, inactivo, apático. Algo que ha puesto en práctica hasta con los periodistas que la escuchaban.

Para ello, para conseguir ese objetivo fundamental, el docente necesita cinco resortes más:

-Evaluar de un modo constructivo, dando apoyo.

-Llevar la clase de una forma cohesiva, como lo haría un buen dirigente de empresa.

-Tener una concepción integradora, conjunta, de la enseñanza y del aprendizaje.

-Emplear métodos de trabajo flexibles y variables.

-Crear entornos de aprendizaje que inspiren.

Experiencia en otros lugares del mundo

Como se ve, son líneas generales que se podían aplicar tanto a una profesora de Uruguay, que se quejaba de que en su país compraban ordenadores para escuelas a donde todavía no había llegado la luz eléctrica, y mucho menos la conexión a internet de banda ancha; como a otra de Mozambique, para quien citar tanto las nuevas tecnologías le rechinaba, porque en Mozambique a lo mejor las escuelas no tienen ni paredes: era como querer que “la Navidad llegue en julio”.

Para hacerse una idea cabal del ambiente en el aula, hay que mencionar también la intervención de un delegado de Senegal. Subrayaba que si un profesor no tiene conciencia de haber sido creado por Dios y no lo transmitía, difícilmente podía ser un buen docente. Una frase que a un representante de Nueva Zelanda, pero anglosajón de raza, parecía sonarle a chino.

Después de un trabajo en equipo y tras haber invitado a todos a contar su idea del profesor del futuro, Tarvainen ha pedido los correos electrónicos de los presentes y ha declarado con solemnidad: “Yo también voy a hacer los deberes, les pasaré un resumen de la clase”.
