Objectives The purpose of this study was to assess whether African-American women are at increased risk of having peripartum cardiomyopathy. Background Peripartum cardiomyopathy is a heart disease of unknown cause that affects young women, often with devastating consequences. The frequency of peripartum cardiomyopathy varies markedly between African and non-African regions. Methods A case-control study was performed at a regional center that provides medical care to a racially heterogeneous population. For each case, 3 healthy control patients were randomly selected who delivered babies within the same month. Results African-American women had a 15.7-fold higher relative risk of peripartum cardiomyopathy than non-African Americans (odds ratio [OR]: 15.7, 95% confidence interval [CI]: 3.5 to 70.6). Other significant univariate risk factors were hypertension (OR: 10.8, 95% CI: 2.6 to 44.4), being unmarried (OR: 4.2, 95% CI: 1.4 to 12.3), and having had _2 previous pregnancies (OR: 2.9, 95% CI: 1.1 to 7.4). African-American ethnicity remained a significant risk factor for peripartum cardiomyopathy when other risk factors were considered in multivariable (OR: 31.5, 95% CI: 3.6 to 277.6) and stratified analyses (OR: 12.9 to 29.1, p _ 0.001). Although the frequency of peripartum cardiomyopathy (185 of 100,000 deliveries) at this center was higher than in previous U.S. reports, it was comparable to the frequency in countries with more women of African descent (100 to 980 of 100,000). Analysis of other U.S. studies confirmed that the frequency of peripartum cardiomyopathy was significantly higher among African-American women. Conclusions African-American women have significantly higher odds of having peripartum cardiomyopathy that could not be explained by several other factors. Further research will be necessary to determine the potential environmental and/or genetic factors associated with African descent that confer this risk. © 2010 by the American College of Cardiology Foundation.

Abstract
Peripartum cardiomyopathy (PPCM) is a rare complication that occurs between the late stage of pregnancy and six months after delivery. PPCM presents as symptoms of left ventricular dysfunction and it can be fatal unless treated promptly. Furthermore, anesthesia and surgery in such patients is a large challenge to anesthesiologists. First and foremost, the maintenance of stable hemodynamics is a major concern. We report a case of combined lumbar epidural anesthesia and both ilioinguinal and iliohypogastric nerve block under ultrasound guided for a wound revision in a 37-year-old woman diagnosed with PPCM after an emergency cesarean section.

Abstract
This case represents the first reported use of levosimendan, a calcium-sensitizing drug with additional vasodilation properties, for treatment of a woman with peripartum cardiomyopathy. Levosimendan induced a steady decline of increased pulmonary capillary wedge pressure, followed by a definitive increase in cardiac stroke volume. The patient recovered from this serious episode of heart failure, and she ultimately regained left ventricular function. IMPLICATIONS: Acute heart failure is a life-threatening event that only rarely occurs during childbirth. We report a case of a woman who experienced cardiovascular collapse during vaginal delivery. We determined that she met current diagnostic criteria for peripartum cardiomyopathy, and we successfully treated her with levosimendan, a calcium-sensitizing drug with additional vasodilation properties.

Background and Objectives : Obstructive sleep apnea syndrome (OSAS) is a common disorder characterized by numerous episodes of absence of respiratory flow during sleep, which can be followed by a decrease in SaO2 , which is rapidly normalized when ventilation resumes. We hypothesize that this hypoxia-reoxygenation phenomena may affect the generation of vascular endothelial growth factor (VEGF), erythropoietin (EPO), endothelin-1 (ENDO-1), and inducible nitric oxide synthase (iNOS). Design and Setting : Prospective, patients referred to sleep disorders center. Patients and Methods : The presence and severity of OSAS were determined using the standard overnight polysomnography. Diagnosis of OSAS was made when the apnea-hypopnea index (AHI) was ≥15, independent of the appearance of symptoms. Serum levels of VEGF, EPO, ENDO-1, and nitrite-nitrate were measured after overnight fasting in 69 patients with OSAS and in 17 healthy control subjects. Serum levels of VEGF and nitrite-nitrate were measured again after 12 weeks of treatment with continuous positive airway pressure (CPAP) in OSAS patients. Results : Serum VEGF levels were found to be significantly higher and nitrite-nitrate levels were found to be significantly lower in OSAS patients than in controls (P=.003, .008, respectively), but no differences in EPO and ENDO-1 levels were found between the groups. We demonstrated that in OSAS patients, the serum VEGF levels were decreased and nitrate levels were increased after 12 weeks of CPAP treatment (P=.001, .002, respectively). Conclusion : According to our data, it is likely that hypoxia-reoxygenation phenomena affect the VEGF and nitrite-nitrate levels, which may be pathogenic factors in generating cardiovascular complications in OSAS.

Introducción
Estudiar la prevalencia de esteatosis hepática no alcohólica (EHNA) en una población pediátrica obesa y su relación con parámetros clinicoanalíticos de resistencia a la insulina (RI) y con niveles de adiponectina.

Pacientes y métodos
Se estudió a 290 niños (4–18 años) con obesidad. Se analizó el índice de masa corporal (IMC) y la desviación estándar (DE), la acantosis nigricans, la presión arterial, el perfil lipídico, las aminotransferasas y la adiponectina. Se realizó un test de sobrecarga oral de glucosa y se definió la RI mediante el homeostasis model assessment (HOMA) y la esteatosis hepática por ecografía. Definimos el síndrome metabólico (SM) como la presencia de 3 o más de los siguientes criterios: obesidad, hipertensión, hipertrigliceridemia, colesterol ligado a lipoproteínas de alta densidad (cHDL) bajo y alteración hidrocarbonada.

Resultados
Cincuenta y dos (18%) pacientes presentaban EHNA en la ecografía, 22 (8%) pacientes tenían elevación de alanin-aminotransferasa (ALT) (≥40U/l). Los pacientes con EHNA fueron significativamente mayores (12,2±2,4 frente a 11,1±2,9 años), más obesos (IMC±DE: 4,5±1,5 frente a 3,8±1,3), y presentaron niveles más elevados de HOMA (3,7±1,5 frente a 2,4±1,4) que los pacientes con ecografía normal. No se encontró diferencia significativa en sexo, raza y estadio puberal. La prevalencia del SM y la acantosis nigricans fue significativamente mayor en los niños con EHNA. Encontramos una correlación inversa entre los niveles séricos de adiponectina y edad, HOMA, ALT y ácido úrico, y encontramos una correlación directa con cHDL. En el análisis de regresión múltiple, las variables que se asociaron de forma independiente con la EHNA fueron IMC±DE, HOMA y adiponectina (odds ratio: 1,4 [1,1-1,9]; 1,3 [1,1-1,6] y 0,9 [0,8-0,9], respectivamente).

Conclusiones
La obesidad y la RI son factores de riesgo de la EHNA en niños y adolescentes. Los niveles bajos de adiponectina están fuertemente asociados al desarrollo de EHNA en dichos pacientes.
