Manejo quirúrgico y rehabilitación de una fractura de olécranon en un paciente con trombocitopenia y ausencia de radio: reporte de un caso

Una mujer de 29 años, mano derecha dominante con síndrome de radio ausente de trombocitopenia, fue tratada quirúrgicamente por una fractura de olécranon derecho. Dada su deformidad en el antebrazo y el brazo de palanca corto, se utilizó una férula correctiva para flexión del codo modificada a medida durante la rehabilitación. Se logró la unión radiográfica. Aunque las puntuaciones de los resultados funcionales fueron similares a las informadas previamente en la población general, el paciente tenía una contractura de extensión persistente.



A pesar de los buenos resultados funcionales, el paciente tenía un rango de movimiento reducido. Con una deformidad severa de la extremidad, el mayor desafío es proporcionar protocolos de rehabilitación óptimos y herramientas individualizadas.

The book "Thrombocytopenia" certainly raises some important issues in its pathogenesis and management. The authors have done a lot of hard work to write state-of-the-art chapters. Each and every chapter is peer-reviewed, evidence-based, and remarkably excellent. Some of the causes of thrombocytopenia are well explained in textbooks, but the topics included in this book are not usually so well described. The chapters are written in such a manner so as to stimulate and keep the reader well-informed. Such an approach is certainly beneficial when aiming to motivate discussion, interaction, innovation, and research. Chapters like "Interferon-Induced Thrombotic Microangiopathy" have been included with the aim to help in understanding the immune pathogenesis of thrombocytopenia. Others have also been selected to keep an eye on the future.

Abstract
Beyond haemostasis, platelets have emerged as versatile effectors of the immune response. The contribution of platelets in inflammation, tissue integrity and defence against infections has considerably widened the spectrum of their role in health and disease. Here, we propose a narrative review that first describes these new platelet attributes. We then examine their relevance to microcirculatory alterations in multi-organ dysfunction, a major sepsis complication. Rapid progresses that are made on the knowledge of novel platelet functions should improve the understanding of thrombocytopenia, a common condition and a predictor of adverse outcome in sepsis, and may provide potential avenues for management and therapy.

Abstract
Thrombocytopenia (TP) affects 7-10% of pregnant women. It occurs 4 times more frequently in pregnancy than in the non-pregnant women population. Women with thrombocytopenia in pregnancy are a heterogeneous and poorly known group. There are several possible causes of thrombocytopenia in pregnancy. The most common are: gestational thrombocytopenia (GE) (60-75%), preeclampsia (PE) and HELLP(hemolysis, elevated liver enzymes, low platelets) syndrome associated TP (21%), and idiopathic immune thrombocytopenia (ITP) (3-10%). Although thrombocytopenia diagnosed in pregnancy in most cases has a mild course, it has also been reported to be associated with a higher rate of preterm birth and premature detachment of the placenta. Some cases of severe thrombocytopenia with systemic involvement are associated with high risk of serious perinatal complications and require early diagnosis, careful clinical monitoring and medical treatment. The differential diagnosis and proper assessment of clinical risk of TP during pregnancy may be of great concern. The article discusses these issues, focusing on pathophysiology of TP in pregnancy.
