Abstract
BACKGROUND:
Although anterior cruciate ligament (ACL) reconstruction is generally regarded as a successful procedure, only 65% of patients return to their preinjury sport. While return-to-sport rates are likely higher in younger patients, there is a paucity of data that focus on the younger patient and their return-to-sport experience after ACL reconstruction.
PURPOSE:
To investigate a range of return-to-sport outcomes in younger athletes who had undergone ACL reconstruction surgery.
CONCLUSION:
A high percentage of younger patients return to their preinjury sport after ACL reconstruction surgery. For patients in this cohort who had not sustained a second ACL injury, the majority continue to participate and are satisfied with their performance.

ANTECEDENTES:
Aunque la reconstrucción del ligamento cruzado anterior (LCA) se considera generalmente como un procedimiento exitoso, sólo el 65% de los pacientes vuelven a su deporte pre-lesión. Aunque las tasas de regreso al deporte son probablemente más altas en los pacientes más jóvenes, hay una escasez de datos que se centran en el paciente más joven y su regreso al deporte después de la reconstrucción del LCA.
PROPÓSITO:
Investigar una gama de resultados de retorno al deporte en atletas más jóvenes que habían sido sometidos a una cirugía de reconstrucción del LCA.
CONCLUSIÓN:
Un alto porcentaje de pacientes más jóvenes vuelven a su deporte pre-lesión después de la cirugía de reconstrucción del LCA. Para los pacientes de esta cohorte que no habían sufrido una segunda lesión del LCA, la mayoría continúa participando y está satisfecha con su desempeño.

Abstract
TITLE:
Incidence of delayed diagnosis of orthopaedic injury in pediatric trauma patients
OBJECTIVE:: Determine the incidence of the delayed diagnosis of orthopaedic injuries in pediatric trauma patients.
DESIGN:
Cross-sectional retrospective analysis.
SETTING:
Level I pediatric trauma center.
PATIENTS/PARTICIPANTS:
All patients with an orthopaedic consultation following a trauma activation with a diagnosis of fracture, dislocation, traumatic arthrotomy, neurovascular injury, amputation, and tendon or ligament injury requiring intervention. A total of 1,009 trauma codes and alerts occurred during the study period, of which 196 patients were diagnosed with an orthopaedic injury.
INTERVENTION:
Charts were reviewed to obtain demographic information, time of presentation, Glasgow Coma Score (GCS) on presentation, injury severity score (ISS), mechanism of injury (MOI), intubation status, length of intensive care unit (ICU) and hospital stay, primary and secondary survey diagnoses, discharge diagnoses, time of additional diagnoses, and reason for delayed diagnosis.
MAIN OUTCOME MEASURES:
Incidence of delayed diagnosis of injury (DDI).
CONCLUSIONS:
In our study, factors that were associated with a DDI included lower GCS, higher ISS, and greater hospital length of stay. There was only one missed injury that required surgical intervention and the remainder were treated conservatively. The initial evaluation of the trauma patient is able to detect life-threatening injuries, but the tertiary survey remains an important part of patient care to detect missed injuries.

OBJETIVO :: Determinar la incidencia del diagnóstico tardío de lesiones ortopédicas en pacientes pediátricos traumatizados.
DISEÑO:
Análisis retrospectivo transversal.
AJUSTE:
Nivel I centro de trauma pediátrico.
PACIENTES / PARTICIPANTES:
Todos los pacientes con una consulta ortopédica tras una activación traumática con un diagnóstico de fractura, luxación, artrotomía traumática, lesión neurovascular, amputación y lesión del tendón o ligamento que requiere intervención. Un total de 1.009 códigos de trauma y alertas ocurrieron durante el período de estudio, de los cuales 196 pacientes fueron diagnosticados con una lesión ortopédica.
INTERVENCIÓN:
Se revisaron los gráficos para obtener información demográfica, tiempo de presentación, puntuación de Coma de Glasgow (GCS) en la presentación, puntuación de la gravedad de la lesión (ISS), mecanismo de lesión (MOI), estado de intubación, duración de la unidad de cuidados intensivos y estancia hospitalaria, Diagnósticos de encuesta primaria y secundaria, diagnósticos de alta, tiempo de diagnósticos adicionales y razón para el diagnóstico tardío.
LAS PRINCIPALES MEDIDAS:
Incidencia del diagnóstico tardío de lesión (DDI).
CONCLUSIONES:
En nuestro estudio, los factores que se asociaron con un DDI incluyeron GCS inferior, ISS más alto, y mayor duración hospitalaria de la estancia. Sólo hubo una lesión faltante que requirió intervención quirúrgica y el resto se trató de manera conservadora. La evaluación inicial del paciente traumatizado es capaz de detectar lesiones que amenazan la vida, pero la encuesta terciaria sigue siendo una parte importante de la atención del paciente para detectar lesiones omitidas.
