The Bullying Culture of Medical School

Powerfully built and with the face of a boxer, he cast a bone-chilling shadow wherever he went in the hospital.
At least that is what my medical school classmates and I thought whenever we passed by a certain resident, or doctor-in-training, just a few years older than we were.
With the wisdom of hindsight, I now see that the young man was a brilliant and promising young doctor who took his patients’ conditions to heart but who also possessed a temper so explosive that medical students dreaded working with him. He had called various classmates “stupid” and “useless” and could erupt with little warning in the middle of hospital halls. Like frightened little mice, we endured the treatment as an inevitable part of medical training, fearful that doing otherwise could result in a career-destroying evaluation or grade.
But one day, one of our classmates, having already been on the receiving end of several of this doctor’s tirades, shouted back. She questioned one of his conclusions in front of the rest of the medical team, insisted on getting an explanation, then screamed back when he started yelling at her.
The entire episode unnerved us all; and over the next few weeks, we marveled at her courage and fretted over her potentially ruined career prospects. But there was one aspect of the event that disturbed us even more. One classmate who had witnessed the “screaming match” described how our fellow medical student had raised her voice and positioned her body as she threatened the doctor. “It was weird,” he recounted. “It was like watching her turn into him.”
For 30 years, medical educators have known that becoming a doctor requires more than an endless array of standardized exams, long hours on the wards and years spent in training. For many medical students, verbal and physical harassment and intimidation are part of the exhausting process, too.
It was a pediatrician, a pioneer in work with abused children, who first noted the problem. And early studies found that abuse of medical studentswas most pronounced in the third year of medical school, when students began working one on one or in small teams with senior physicians and residents in the hospital. The first surveys found that as many as 85 percent of students felt they had been abused during their third year. They described mistreatment that ranged from being yelled at and told they were “worthless” or “the stupidest medical student,” to being threatened with bad grades or a ruined career and even getting hit, pushed or made the target of a thrown medical tool.
Nonetheless, many of these researchers believed that such mistreatment could be eliminated, or at least significantly mitigated, if each medical school acknowledged the behavior, then created institutional anti-harassment policies, grievance committees and educational, training and counseling programs to break the abuse cycle.
One medical school became a leader in adopting such changes. Starting in 1995, educators at the David Geffen School of Medicine at the University of California, Los Angeles, began instituting a series of schoolwide reforms. They adopted policies to reduce abuse and promote prevention; established a Gender and Power Abuse Committee, mandated lectures, workshops and training sessions for students, residents and faculty members; and created an office to accept confidential reports, investigate and then address allegations of mistreatment.
To gauge the effectiveness of these initiatives, the school also began asking all students at the end of their third year to complete a five-question survey on whether they felt they had been mistreated over the course of the year.
The school has just published the sobering results of the surveys over the last 13 years. While there appears to have been a slight drop in the numbers of students who report experiencing mistreatment, more than half of all medical students still said that they had been intimidated or physically or verbally harassed.
Students described being yelled at, pushed and threatened. One student recounted being slapped on the hand by a more senior doctor who said, “If teaching doesn’t help you learn, then pain will.” Some students wrote about racial insults, with senior staff members making noises to mimic a foreign language; others reported being grabbed, asked out on a date or passed over because of their sex.
“We were really crushed when we saw the results,” said Joyce M. Fried, lead author of the paper and assistant dean and chairwoman of the Gender and Power Abuse Committee at the medical school. “We were disappointed that it was so difficult to change.”
U.C.L.A.’s experience is not isolated. In fact, national medical education surveys that include questions about mistreatment indicate that the environment at that school is about average. And the striking similarity of experiences across a generation of students suggests problems not just with one institution, but with the culture of medical training itself. “This is a national problem,” Ms. Fried said. “Our faculty and doctors-in-training come from all over, including schools where some of them might have been mistreated.”
While their findings are disheartening, Ms. Fried and her colleagues continue to believe that medical student mistreatment can be significantly reduced — but only if all medical schools come together to work on the issue. “We’re talking about the really hard task of changing a culture, and that has to be done on a national level,” Ms. Fried said. Such an effort would include shared training programs, common policies regarding mistreatment and greater transparency about the mistreatment that currently exists in medical schools.
“There are a lot of really good people and role models out there,” Ms. Fried said. “But the culture for all these years has been to just take the mistreatment and not say anything.”
“It wasn’t right back then, and it shouldn’t be tolerated anymore,” she added.

Para Sobrevivientes de Polio
¿Sabía usted que existe de 12 a 20 millones de individuos que han tenido polio viviendo en el mundo el día de hoy? Aunque algunas personas piensan que "la polio ya no existe más" el virus de la polio aún hoy en día sigue causando poliomielitis en niños que no han sido vacunados en forma apropiada. Usted puede vivir en un país que todavía está tratando de erradicar el virus de la polio o en el cual hay barreras de educación, empleo y cuidado de la salud para sobrevivientes de la polio. O puede vivir en un lugar donde los casos de polio aguda ya no ocurran, pero en donde usted tiene que retar al status quo que excluye a las personas con discapacidades de una participación plena en la vida. En cualquier caso, para usted, la polio aún existe.
Existen consecuencias físicas de largo plazo para aquellas personas que han tenido poliomielitis. Los nuevos síntomas reconocidos por la comunidad médica y que pueden estar relacionados con haber tenido polio con anterioridad incluyen:
- Fatiga inusual – cansancio rápido de los músculos o sensación de fatiga intensa;
- Nueva debilidad muscular, tanto en aquellos músculos previamente afectados por la polio y en los que parecían no afectados;
- Dolor en los músculos y/o las articulaciones;
- Problemas para dormir; respirar o tragar; y/o
- Disminución de la habilidad para tolerar temperaturas bajas.
Cualquier combinación de estos síntomas puede afectar su habilidad de realizar actividades cotidianas tales como: subir escaleras, caminar, levantar, etc.
Acciones Recomendadas
Obtenga una evaluación general completa de su médico de cabecera. Cualquier problema médico que sea encontrado durante la evaluación deberá ser tratado y monitoreado.
Si sus síntomas persisten, sométase a una evaluación neuromuscular por parte de un especialista en problemas de síndrome post-polio, usualmente un especialista en medicina física y rehabilitación (fisioterapista) o un neurólogo. El propósito de esta evaluación es el de determinar cuál es el problema y el de establecer una línea de referencia con la cual se puedan comparar los cambios futuros, para evaluar y actualizar los aparatos que le ayuden a movilizarse y/o equipo ventilatorio o para desarrollar un plan de tratamiento individualizado.
No trate de diagnosticarse usted mismo. El diagnóstico del síndrome de post-polio es uno de exclusión y es importante descartar (o tratar) las enfermedades con síntomas similares que puedan estar causando la aflicción.
Aunque las investigaciones aún no han identificado un medicamento que evite o retrase el debilitamiento de los músculos, los síntomas pueden ser controlados.
- Consulte con un médico y utilice los aparatos de movilización y equipo ventilatorio que le hayan sido recomendados.
- Preste atención a las señales que da su cuerpo y siga las pautas de sentido común que incluyen evitar las actividades que causen dolor y/o fatiga y que duren más de diez minutos.
- Tómese su tiempo al realizar las actividades cotidianas, suspendiendo para descansar cada 15 ó 30 minutos varias veces al día.
- Considere unirse a un grupo de apoyo que promueva la auto ayuda e incentive la participación grupal y la acción positiva y/o considere buscar asesoría individual o familiar que le ayude a realizar los cambios necesarios en su estilo de vida.
Mantener la salud es esencial para aquellos con un sistema neuromuscular comprometido. Preste atención a los consejos generales de una buena alimentación, dormir lo suficiente, evitar hábitos no saludables como el fumar y comer en exceso y ejercitarse de manera apropiada. Muchos síntomas resultan como consecuencia del mal uso y abuso de los músculos y de las articulaciones. Considere con cuidado qué músculos va a ejercitar y con qué frecuencia.
Edúquese usted mismo y eduque a sus seres amados sobre los problemas de post -polio y los métodos de tratamiento. Mantenga una relación con un equipo de profesionales médicos tradicionales y cuando sea apropiado, con otros profesionales de salud que los complementen, al colaborar con todos ellos usted lucha por su salud y por su independencia.
Para Familiares y Amigos
La experiencia de haber tenido poliomielitis (parálisis infantil) es una experiencia muy personal. El tener la forma aguda de la enfermedad puede resultar en la no-hospitalización o el aislamiento por meses o tal vez hasta años de rehabilitación. Así mismo, los recuerdos de la polio varían. Aquellos sobrevivientes que sufrieron de polio cuando eran niños quizás pueden no tener recuerdos conscientes de la enfermedad o de la subsiguiente rehabilitación, mientras que otros sobrevivientes recuerdan vivamente el dolor y la parálisis de la infección aguda y sus tratamientos médicos. La atención médica varía de país en país, de una década a otra década y de comunidades rurales a la ciudad.La poliomielitis es causada por cualquiera de los tres serotipos del virus de la polio. Los tipos principales de la enfermedad incluyen poliomielitis paralítica y la no-paralítica. El virus de la polio daña las células nerviosas motoras, como consecuencia, los músculos que son instruidos por aquellos nervios son paralizados tanto de manera temporal o permanente. La polio aguda no infecta los nervios sensores, por esto la sensibilidad no se ve afectada. La poliomielitis paralítica puede ser clasificada como espinal, bulbar o bulboespinal. La bulbar es la forma más grave de la polio y afecta a la parte del cerebro que se encarga de las funciones vitales de respirar y espacio.Los nuevos problemas que su ser amado enfrenta no son causados primordialmente por el virus de la polio, sino que son trastornos secundarios como consecuencia de haber tenido polio. Los sobrevivientes de polio que se encuentren experimentando nuevos síntomas no han sido infectados nuevamente y no son contagiosos. Ellos necesitan su comprensión y su espacio. Post-Polio Health International recomienda que todos los sobrevivientes de polio se hagan exámenes médicos anuales y que aquellos que estén sufriendo síntomas de debilidad, dolor, fatiga, y problemas respiratorios obtengan una evaluación neuromuscular exhaustiva. Algunos sobrevivientes se rehúsan a buscar atención médica. Algunas de las razones para evadir la visita a un médico pueden incluir malos recuerdos de la polio, la creencia que ningún médico hoy en día entiende los efectos tardíos de la polio, una visita reciente que no ayudó mucho o una oficina o mesa de examinación inaccesible.
Su rol como miembro de la familia puede ser el de incentivar una evaluación y ayudar a la implementación de un plan de tratamiento.
Los sobrevivientes de polio si o no sufren sus nuevos problemas en aislamiento. Lo que les afecta a ellos puede afectarle a usted. Usted posiblemente estará afrontando por primera vez muchos asuntos:
Seleccionar profesionales de la salud especializados y equipos nuevos;
Aprender sobre leyes de acceso y discapacidad (access and disability laws);
Tomar decisiones de còmo utilizar su tiempo, energía y recursos financieros.
De manera adicional, algunos roles asumidos previamente quizás deberán ser cambiados o disputados debido a la necesidad.
Entender y aceptar todas las ramificaciones de los efectos tardíos de la polio no es fácil. Nadie puede predecir el futuro, pero se recomienda que usted y aquel sobreviviente de polio en su vida intenten abordar las preocupaciones como un equipo que incluya a profesionales de la salud.
Los sobrevivientes de polio experimentan sentimientos de culpa; ellos temen ser una "carga" o, pueden sentir tristeza por la "forma en la que las cosas sucedieron." Usted quizás podrá sentir resentimiento por sus nuevas responsabilidades asumidas. Se recomienda que usted enfrente estos asuntos de manera en que surjan. Todos tenemos habilidades, métodos y preferencias diferentes para resolver problemas. Descubrir y utilizar esas habilidades puede ser muy beneficioso para mantener una relación saludable. Muchas relaciones se han beneficiado de una asociación con un grupo de apoyo o de auto ayuda o de asesoramiento familiar.
Educarse usted mismo de las muchas facetas de los efectos tardíos de la polio es el primer paso. Para determinar su rol en un plan de tratamiento cuyo fundamento sea el compromiso de que su ser amado realice cambios en su estilo de vida.
Para Profesionales de la Salud
Sobrevivientes de polio pueden buscar su consejo médico sobre una nueva debilidad, fatiga agobiante y/o dolor. Algunos pacientes pueden describir estos síntomas y "olvidarse" de mencionar que ellos han tenido polio. Este trío de síntomas típicamente se presenta por lo menos después de quince años de un caso agudo de poliomielitis, como lo demuestra la experiencia documentada en América de Norte, Europa Occidental y Australasia. Individuos, quienes ahora se encuentran en su séptima u octava década, están enfrentando una combinación de nuevos problemas de polio y envejecimiento. Debido a que la poliomielitis aún no ha sido erradicada del mundo, sus sobrevivientes van a buscar ayuda por muchos años futuros.
En 1875, Raymont y Charcot describieron un paciente de polio que les informó de una debilidad y atrofia en su brazo derecho- el brazo que él usaba excesivamente debido a una debilidad residual en su brazo izquierdo. Mientras que sobrevivientes de la epidemia de 1950 buscaban atención médica por "cansarse con mayor facilidad" los investigadores exploraron estas nuevas quejas y a través de los años, han desarrollado los siguientes criterios del síndrome de post-polio:*
- Un caso de poliomielitis paralítica previo con evidencia de pérdida de neuronas motoras, confirmada por una enfermedad paralítica aguda, señales de debilidad y atrofia muscular durante un examen neurológico y signos de denervación en una electromiografía (EMG)
- Un período de recuperación funcional completo o parcial después de una poliomielitis paralítica aguda, seguida de un intervalo (usualmente 15 años o más) de función neurológica estable.
- Aparición gradual o progresiva y persistente de nueva debilidad muscular o fatiga muscular anormal. (Disminución de la resistencia), con o sin fatiga generalizada, atrofia muscular, o dolor muscular o de las articulaciones. (La aparición repentina puede seguir a un periodo de inactividad, o trauma o cirugía)
- Síntomas menos comunes, que son atribuidos a problemas del síndrome de post-polio incluyennuevos problemas al respirar o al tragar.
- Síntomas que persisten al menos por un año.
- Exclusión de otros problemas neurológicos, médicos y terapéuticos como causantes de los síntomas.
Es importante el darse cuenta de que existen otras consecuencias por haber tenido polio que no corresponden a este criterio. Los sobrevivientes de polio que visiten su oficina pueden tener una variedad de quejas sobre problemas neurológicos, ortopédicos, médicos, musculares y del esqueleto, emocionales, y de rehabilitación, todos los cuales deberán ser enfrentados en forma metódica y no ser considerados como simples señales de envejecimiento.
Post-Polio Health International recomienda que todos los sobrevivientes de polio reciban constantes evaluaciones médicas básicas. Si los síntomas del paciente no pueden ser explicados ni aliviados por los enfoques de la medicina general y si los síntomas persisten o empeoran, se necesitará la derivación a un especialista. Un psiquiatra o un neurólogo pueden realizar una evaluación neuromuscular para establecer un diagnóstico y recomendar un plan de tratamiento que le será enviado a usted y a su paciente.
