Abstract
In his brief report on the structure of the gray matter of the central nervous system (1873), in which he described the "black reaction", Golgi noted the ramifications of the axon. This discovery prompted the French histologist Louis Antoine Ranvier, one of the first to try the black reaction outside Italy, to propose an ingenious theory of referred pain in his Traité technique d'histologie. Ranvier suggested that the nerve fibers originating from the irritated area and those coming from the region to which the sensation is referred converge on the same axon and thus the same cell body, causing the spatial dislocation of sensation. This theory of referred pain is a powerful example of the extraordinary clinical-physiological impact of the first of Golgi's neurocytological discoveries.

Abstract
On April 1898 Camillo Golgi communicated to the Medical-Surgical Society of Pavia, the discovery of the "internal reticular apparatus", a novel intracellular organelle which he observed in nerve cells with the silver impregnation he had introduced for the staining of the nervous system. Soon after the discovery it became evident that this cellular component, which was also named the "Golgi apparatus", was a ubiquitous structure in eukaryotic cells. However the reality of the organelle was questioned for years and many cytologists considered the internal reticular apparatus as an artefact due to the fixation and/or metallic impregnation procedure. The controversy was finally solved in the mid-1950s by electron microscopy when the Golgi apparatus definitely acquired its dignity of being a genuine cell organelle. The designation of "Golgi complex" entered officially in the literature in 1956. Both the terms Golgi apparatus and Golgi complex are currently interchangeable. However a quick "the Golgi" and the introduction of Golgi in adjectival form are now prevalent in the blooming scientific literature on the organelle. Thus Camillo Golgi underwent his final transformation and, becoming the eponym of the organelle he had discovered, he found a way to immortality.

¿Hay algo más saludable que la dieta mediterránea? Pues sí, la misma dieta tradicional pero con frutos secos o aceite de oliva extra virgen. Lo afirma un estudio español en el que se ha visto que este 'cóctel' saludable, rico en antioxidantes, se asocia con una mejor función cognitiva en personas mayores. En el artículo que publica la revista JAMA Internal Medicine se demuestran los beneficios de la dieta mediterránea para prevenir o retrasar el deterioro cognitivo asociado al envejecimiento que eventualmente termina en demencia o alzhéimer. El estudio, derivado del ensayo PREDIMED, Prevención con Dieta Mediterránea, revela que, después de 4 años de seguimiento, las personas que siguen esta dieta complementada con aceite de oliva virgen extra o frutos secos tienen una mejor función cognitiva que las personas asignadas a la dieta control....

Abstract
The scope of lung ultrasound (LUS) in emergency and critical care settings has been studied extensively. LUS is easily available at bedside, free of radiation hazard and real time. All these features make it useful in reducing need of bedside X-rays and CT scan of chest. LUS has been proven to be superior to the bedside chest X-ray and equal to chest CT in diagnosing many pleural and lung pathologies. The first International Consensus Conference on Lung Ultrasound (ICC-LUS) has given recommendations for unified approach and language in major six areas of LUS. The LUS diagnosis is to be given after integration of findings of both lungs. The BLUE protocol is first LUS-based systematic approach in diagnosing pleural and lung pathologies. The protocol suggested in this article includes history and conventional clinical assessment along with LUS features.

Abstract
OBJECTIVE: Bedside lung ultrasound (LUS) is a noninvasive, readily available imaging modality that can complement clinical evaluation. TheBedside Lung Ultrasound in Emergency (BLUE) protocol has demonstrated a high diagnostic accuracy in patients with acute respiratory failure (ARF). Recently, bedside LUS has been added to the medical training program of our ICU. The aim of this study was to investigate the accuracy of LUS based on the BLUE protocol, when performed by physicians who are not ultrasound experts, to guide the diagnosis of ARF.
METHODS: Over a one-year period, all spontaneously breathing adult patients consecutively admitted to the ICU for ARF were prospectively included. After training, 4 non-ultrasound experts performed LUS within 20 minutes of patient admission. They were blinded to patient medical history. LUS diagnosis was compared with the final clinical diagnosis made by the ICU team before patients were discharged from the ICU (gold standard). RESULTS:
Thirty-seven patients were included in the analysis (mean age, 73.2 ± 14.7 years; APACHE II, 19.2 ± 7.3). LUS diagnosis had a good agreement with the final diagnosis in 84% of patients (overall kappa, 0.81). The most common etiologies for ARF were pneumonia (n = 17) and hemodynamic lung edema (n = 15). The sensitivity and specificity of LUS as measured against the final diagnosis were, respectively, 88% and 90% for pneumonia and 86% and 87% for hemodynamic lung edema. CONCLUSIONS: LUS based on the BLUE protocol was reproducible by physicians who are not ultrasound experts and accurate for the diagnosis of pneumonia and hemodynamic lung edema.

El vino tinto contiene diversos polifenoles que contribuyen a la reducción y eliminación de radicales libres, combate la oxidación, induce apóptosis de determinadas células cancerígenas, es quimiopreventivo.

Y, después de, ojo que no pararé de seguir Aprendiendo, es posible afirmar que el vino tino que bebía la tripulación del Submarino K 19, era una herramienta preventiva ante los efectos ionizantes que estaban recibiendo por su continua proximidad a las fuentes radiactivas. Es muy posible que la investigación en Protección Radiólogica de los años 60, y anteriores, sea mucho menos avanzada que la actual, y las barreras y sistemas de protección radiológica actuales sean mucho mayores en los Submarinos Nucleares. Lamentablemente, beber Vino Tinto, en cantidades moderadas, no puede evitar los efectos letales de una exposición tan alta como la que tuvieron los tripulantes.

Abstract
Objective: Pruritus is a common and disturbing side effect of neuraxial opioids after cesarean section. The purpose of this study was to compare the efficacy of intravenous ondansetron and sub-hypnotic dose of propofol in control and treatment of intrathecal sufentanil induced pruritus in cesarean surgery. Methods: Totally, 90 parturient with American Society of Anesthesiology physical status grade I-II, undergoing spinal anesthesia with 2.5 μg sufentanil and 10 mg bupivacaine 0.5% were enrolled to this randomized, prospective, double-blind study. The women were randomly assigned to two groups who received 8 mg ondansetron or 10 mg propofol to treat pruritus grade ≥3. The patient was evaluated after 5 min and in the lack of successful treatment, the doses of two drugs repeated and if the pruritus is on-going, the exact treatment with naloxone was done. Findings: The incidence of pruritus was 69.3%. Both groups were well-matched. The peak time pruritus was 30-75 min after injection. The percentage of individuals consumed naloxone were 6.8% and 15.9% in ondansetron and propofol groups, respectively (P = 0.18). The mean score of satisfaction (according to visual analog scale criteria) was 9.09 ± 1.1 in ondansetron group and 9.3 ± 1.07 in the propofol group (P = 0.39). Conclusion: Ondansetrone and sub-hypnotic dose of propofol are both safe and well-tolerated. Due to their same efficacy in the treatment of intrathecal sufentanil-induced pruritus, they can be widely used in clinical practice.Keywords: Caesarean surgery; intrathecal opioid; Ondansetron; Propofol; pruritus

Abstract
The objective of this study was to assess the impact of increased pre-pregnancy maternal body mass index (BMI) on perinatal outcomes in term, singleton pregnancies who received prenatal care in community-based practices. The sample of 1996 infants included in the study was drawn from the All Our Babies Study, a prospective pregnancy cohort from Calgary. Multivariable logistic regression explored the relationship between the main outcomes, infant birth weight, Apgar score, admission to neonatal intensive care (NICU) and newborn duration of hospitalization, and BMI prior to pregnancy. Approximately 10% of the infants were macrosoms, 1.5% had a low Apgar score (<7 at 5 min), 6% were admitted to intensive care and 96% were discharged within 48 h after delivery. Although the infants of overweight and obese women were more likely to have increased birth weight as compared to infants of normal weight women, there were no differences in Apgar score, admission to NICU, or length of postnatal hospital stay among groups. This study suggests that in otherwise healthy term, singleton pregnancies, obesity does not seem to increase the risk of severe fetal impairment, neonatal admission to intensive care or duration of postnatal hospitalization.

Abstract
OBJECTIVE: To evaluate incidence of breastfeeding initiation according to maternal pre-pregnancy body mass index (BMI) in "Grossesse en Santé", a large prospective birth cohort in Quebec City. METHODS: Breastfeeding initiation in the post-partum period, pre-pregnancy BMI, sociodemographic determinants and obstetrical and neonatal factors were collected from years 2005 to 2010 in 6592 women with single pregnancies. Prenatal non-intention to breastfeed was documented in a subgroup of the cohort (years 2009-2010). Log-binomial regression analyses were performed to assess relative risk (RR) of non-initiation of breastfeeding between maternal BMI categories in models including pre- and post-natal determinants, after exclusion of variables with a mediating effect. RESULTS: Twenty percent (20%) of obese women did not initiate breastfeeding in the post-natal period at hospital compared to 12% for normal weight women. Compared with those having a normal pre-pregnancy BMI, obese women had a higher risk of non-initiation of breastfeeding (RRunadj 1.69, 95% CI 1.44-1.98), even after adjustment for prenatal and sociodemographic factors (RRadj 1.26, 95% CI 1.08-1.46). Furthermore, the risk of non-initiation of breastfeeding in obese women still remained higher after introduction of per- and post-natal factors (RR 1.22, 95% CI 1.04-1.42). The prenatal non-intention to breastfeed was strongly associated with the non-initiation of breastfeeding for all categories of BMI. CONCLUSION: Maternal obesity is associated with a two-fold rate of non-initiation of breastfeeding. Considering the benefits of breastfeeding and the increasing obesity rate, adapted interventions and specialized support should target both pre- and immediate post-natal periods in this population.

Abstract
BACKGROUND:Obesity is a risk factor for the development of certain respiratory diseases, and neonatal overfeeding results in an early onset ofobesity in adulthood. However, the influence of neonatal overfeeding on respiratory diseases has rarely been studied. Therefore, this paper is aimed at investigating the effect of neonatal overfeeding on airway responsiveness and inflammation.METHODOLOGY/PRINCIPAL FINDINGS: The neonatal overfeeding was induced by reducing litter size to three pups per litter (small litter, SL) in contrast to the normal litter size with ten pups per litter (NL) on postnatal day 3 (P3) in male ICR mice. On P21, mice were weaned to standard chow diet. Airway responsiveness to methacholine was measured either on P21 or P150. Total and classified inflammatory cells in bronchoalveolar lavage fluid (BALF) were counted, lung inflammatory cells were evaluated through staining with hematoxylin & eosin and F4/80 immunohistochemistry; lung fibrosis was evaluated through staining with Masson and α-SAM immunohistochemistry. Leptin levels in serum were measured by RIA; TNF-α levels in serum and BALF were quantified by ELISA; mRNA levels of TNF-α, CTGF and TGF-β1 in lung tissues were measured using real-time PCR. Mice from SL exhibited accelerated body weight gain, impaired glucose tolerance and hyperleptinemia. Enhanced airway responsiveness to methacholine was observed in SL mice on P150, but not on P21. Pulmonary inflammation was evident in SL mice on P150, as reflected by inflammatory cells especially macrophages around bronchi and interstitium. BALF and serum TNF-α levels and lung TNF-α mRNA expression were significantly increased in SL mice on P150. More collagen accumulated surrounding the bronchi on P150; lung mRNA levels of TGF-β1 and CTGF were also increased on P150. CONCLUSION: In addition to inducing a variety of metabolic defects, neonatal overfeeding enhanced lung inflammation, which may lead to airway remodeling and airway hyperresponsiveness in adulthood.
