Manejo de la situación no puedo intubar, no puedo ventilar Management of the 'can't intubate, can't ventilate' situation Leonard Pott Correspondence Email: lpott@psu.edu Update in Anaesthesia | www.anaesthesiologists.org INTRODUCTION Even when a patient proves to be unexpectedly difficult to intubate, it is usually not a problem to adequately oxygenate and ventilate the patient using bag-mask ventilation (BMV). Occasionally, and fortunately very rarely, we encounter a patient who is impossible to intubate AND who also cannot be adequately oxygenated. This is the feared 'can't intubate, can't ventilate' situation. The incidence of 'can't intubate, can't ventilate' in patients who are not expected to be difficult intubations is probably around 1 in 10 000 anesthetics. Tillmann Hein, MD, Michael Foreman, MD AANA Journal October 2009, Vol. 77, No. 5 Our anesthesia care team was called to care for a patient who was admitted to the emergency department with the esophageal-tracheal double-lumen airway device (Combitube, Tyco Healthcare, Nellcor,Pleasanton, California) in place, which needed to be exchanged for a definitive airway because the patient required an extended period of mechanical ventilation. Several techniques were attempted to exchange the esophageal-tracheal Combitube (ETC) without success. First, we attempted direct laryngoscopy with the ETC in place after deflation of the No. 1 proximal cuff and sweeping the ETC to the left. We were prepared to use bougie-assisted intubation but could not identify any airway anatomy. After removal of the ETC, we unsuccessfully attempted ventilation/intubation with a laryngeal mask airway (LMA Fastrach, LMA North America, San Diego, California). Our third attempt was insertion of another laryngeal mask airway (LMA Unique, LMA North America) with marginal ventilation, but we again experienced unsuccessful intubation using a fiberscope. The ETC was reinserted after each intubation attempt because mask ventilation was impossible. Before proceeding with cricothyrotomy, we repeated direct laryngoscopy but without the ETC in place. We identified the tip of the epiglottis, which allowed for bougie-assisted intubation. This obviated the need for emergency cricothyrotomy. CODEX AUREUS

El Codex Aureus, también llamado Codex Aureus de San Emmeram, es un manuscrito con los cuatro evangelios, que fue confeccionado en el año 870 en Francia por encargo del rey Carlos II el Calvo. Presenta un formato de 42 por 33 cm, con una extensión de 126 hojas, escritas en latín, 7 miniaturas a página completa, 12 tablas de concordancia y 10 páginas decorativas; en el siglo X se añadió una octava miniatura con la imagen de un abad.

Este códice fue realizado para el uso personal del rey Carlos II de Francia, a finales del siglo IX. Según algunos documentos de Ratisbona del siglo XI, fue donado en el 893 por el rey Arnulfo al importante monasterio de San Emmeram en Ratisbona, donde fue restaurado entre los años 975 y 1001, durante este tiempo se agregó la lámina con el retrato de Ramwold, abad de dicho monasterio.

Después de esto, el códice llego en 1811 a la biblioteca real de Múnich, cuya colección se integro posteriormente a la Bayerische Staatsbibliothek, donde se encuentra hoy en día.Este códice es uno de los últimos ejemplares manufacturados por la escuela carolingia.

Contiene los textos de los cuatro evangelistas, cada uno con su prefacio; está escrito a dos columnas con letras de oro y escritura uncial, siguiendo el modelo de la escuela Palatina de Carlomagno. En el prólogo de este documento se elogia al rey donante; además la página de dedicación señala a los hermanos Berengario y Liuthard como copistas, el año de conclusión del códice (875) y al rey Carlos, donante del libro.

Todas las columnas de texto se encuentran decoradas con marcos, los cuales son diferentes unos de otros. La exuberante decoración se acentúa con innumerables letras capitulares y diez páginas decorativas, dos al principio de la obra y dos antes de cada evangelio. En las láminas completas, se representan a los cuatro evangelistas –cada uno en una hoja separada- , un Majestas Domini, una adoración del cordero y la dedicatoria con el rey Carlos en el trono. Las tapas originales de este libro, confeccionadas en oro repujado, filigrana y piedras preciosas, constituyen uno de los más importantes tesoros de la época medieval, presentan al centro una imagen de Jesús en el trono, y en los espacios laterales, la imagen de los cuatro evangelistas y otros textos bíblicos. The results obtained during this pandemic, more than any randomized trial, led to the worldwide acceptance of the use of membrane lungs. Moreover, as centers that applied this technique as rescue therapy for refractory hypoxemia recognized its strength and limited technical challenges, the indications for ECMO have recently been extended. Indications for veno-venous ECMO currently include respiratory support as a bridge to lung transplantation, correction of lung hyperinflation during chronic obstructive pulmonary disease exacerbation and respiratory support in patients with the acute respiratory distress syndrome, possibly also without mechanical ventilation.
