Por primera vez en 29 años de existencia, una mujer presidirá la Sociedad Mexicana de Ortopedia Pediátrica (SMOP), será la tabasqueña Sonia Castro Noriega, quien ha presentado una propuesta de ley ante el Congreso de la Unión para que por decreto a todos los niños recién nacidos y menores de cuatro meses, se les realice un radiografías para descartar la presencia de displasia de cadera.
La especialista en ortopedia y traumatología pediátrica, indicó que la luxación congénita de cadera o displasia del desarrollo de cadera, puede llegar a ser una enfermedad discapitante en la edad joven o adulta, de ahí su importancia para que se realicen las detecciones y correciones a temprana edad.
¿Cómo la nombran Presidenta de la Sociedad Mexicana de Ortopedia Pediátrica?
El año pasado me eligieron vicepresidenta y de acuerdo a los estatutos, este año rendiría protesta como presidenta, y así fue el pasado 17 de noviembre en nuestra asamblea anual, realizada en Campeche; a mí me proponen mis maestros y compañeros del hospital Shriners, donde hice mi especialidad.
¿Qué significado tiene esta distinción?
Para mí es de mucho orgullo, una porque soy la primer mujer que preside a la Sociedad, fui elegida entre 400 miembros, y otra porque soy tabasqueña y nos gusta que nos distingan a nivel nacional.
¿Qué la hace meritoria a este reconocimiento?
Aquí se valora o se evalúa el desempeño que cada uno de los agremiados de la SMOP ha tenido a lo largo de los años, el ejercicio a nivel hospitalario, las intervenciones que tenemos, la investigación, en los congresos, afortunadamente y gracias a la pasión que tengo por mi especialidad he tenido la oportunidad de participar en varios congresos nacionales e internacionales con ponencias, simposium y conferencias.
Además junto con cuatro colegas, elaboramos el plan nacional de detección oportuna de displasia del desarrollo de la cadera, conocido como luxación congénita de la cadera, que puede llegar a ser una enfermedad discapacitante; ya lo terminamos y posiblemente durante mi gestión nos toque ponerlo en marcha.
¿Ya lo han planteado a la Secretaría de Salud Pública o por donde empezarán?
Te adelantó que a nivel federal ante la Comision de Salud de la Cámara de Senadores, emitimos una propuesta para tratar que se genere una iniciativa de ley, por la que se estipule que todos los niños menores de 4 meses de edad se les haga una radiografía o un ultrasonido para estar seguros que no traen luxación de cadera o una displasia de cadera, porque desgraciadamente en nuestro país este padecimiento se detecta cuando el niño camina con dificultad, y está en trámite esta aprobación.
¿Cómo esta Tabasco en el tratamiento de displacia de cadera?
Desafortunadamente en México, no solo en Tabasco, no tenemos estadisticas de cuántas luxaciones de cadera por cada mil nacidos vivos, y eso es uno de los primeros trabajos que la Sociedad Mexicana de Ortopedia Pediatrica nos hemos propuesto, se van a empezar a realizar estudios, como una encuesta a nivel nacinal en cada Estado vamos a empezar a registrar el número de niños con displacia de cadera.
Yo en lo personal, en 16 años de estar trabajando en el hospital del niño como ortopedista, les puedo decir que si es un gran volumen de niños que nos ha tocado ver, y puedo asegurar que el 95 por ciento de las luxaciones de cadera se diagnosticaron a dis tiempo, es decir llegaron ya que el niño caminaba.
¿Porqué es importante que este plan se establezca en México?
Existe una asociación internacional ponseti, del manejo de estos piecitos que es con yesos, ya no con cirujias, se le va corrigiendo la deformidad sin someterlos a cirugias extensas.
La propuesta para implantar este Plan en México ya fue aceptada por la asociacion internacional Ponseti que esta en Iowa en Estados Unidos, revisó nuestro plan, lo acepto, le gustó y ya nos dio el aval y el reconocimiento para suplantación en el país, incluso lo estan tomando de modelo para implantarlo en Latinoamerica.

Abstract
We performed a prospective, double-blinded study in 20 patients undergoing gynecologic surgery with lower abdominal incision, to investigate characteristics of intrathecal hyperbaric levobupivacaine compared with isobaric levobupivacaine. We randomly assigned them to receive either isobaric or hyperbaric 0.42% levobupivacaine intrathecally. We found that hyperbaric levobupivacaine, compared with isobaric levobupivacaine, spread faster to T10 level (2.8 ± 1.1 versus 6.6 ± 4.7 minutes, P = 0.039), reached higher sensory block levels at 5 and 15 minutes after injection (T8 versus L1, P = 0.011, and T4 versus T7, P = 0.027, resp.), and had a higher peak level (T4 versus T8, P = 0.040). Isobaric levobupivacaine caused a wider range of peak levels (L1 to C8) compared with hyperbaric form (T7 to T2). The level of T4 or higher reached 90% in the hyperbaric group compared with 20% in the isobaric group (P = 0.005). Our results suggest that hyperbaric levobupivacaine was more predictable for sensory block level and more effective for surgical procedures with lower abdominal approach. Hyperbaric levobupivacaine seems to be suitable, but the optimal dosage needs further investigation.

Abstract

BACKGROUND:It was aimed to compare the efficacy and adverse effects of levobupivacaine alone and in combination with fentanyl and sufentanil during transurethral resection of the prostate (TURP) under spinal anesthesia.MATERIALS AND METHODS:In this prospective, randomized, double-blind trial, 60 patients undergoing elective TURP under spinal anesthesia were randomized into three groups. Ten milligrams of 0.5% levobupivacaine in Group-I, 7.5 mg 0.5% levobupivacaine combined with 25 μg fentanyl in Group-II and 7.5 mg 0.5% levobupivacaine with 2.5 μg sufentanil in Group-III were administered intrathecally. RESULTS: The time for sensorial block to reach level T10 was 10.2 ± 2.0, 6.9 ± 1.7 and 7.0 ± 1.4 min in Group-I, II and III, respectively (P < 0.001). The maximum sensorial block level was T8 in all groups. The frequency of a complete motor block was higher in Group-I. The mean duration of motor block was shorter in Group-II and III than in Group-I (P < 0.001). There were no differences between groups regarding side effects (P > 0.05). The time for first analgesic request was shorter in Group-I than in the other two groups (P < 0.05). During the first postoperative 24-h period, 11 (58%) patients in Group-I, 9 (48%) patients in Group II and 9 (45%) patients in Group-III required an analgesic drug (P > 0.05). CONCLUSION: This study showed that combining lower dose levobupivacaine with fentanyl and sufentanil provides faster onset of sensorial block, lower frequency and shorter duration of motor block, and longer analgesia time in TURP under spinal anesthesia.

Abstract
Background. The behaviour of isobaric levobupivacaine in relation to gravity when used in obstetric spinal anesthesia is unclear. Methods. 46 women with ASA physical status 1 undergoing cesarean section were randomly allocated to 2 groups. Spinal anesthesia with 12.5 mg levobupivacaine was performed in the sitting position in all women. Those in the first group were placed in the supine position immediately after the injection, while those in the second group were asked to remain seated for 2 minutes before assuming the supine position. The sensory block level, the onset of sensory and motor blocks, the regression of the sensory block for 2 dermatomes of the sensory block, the first request for analgesics, and the regression of motor block were recorded. Results. No differences in onset times, sensory level, or Bromage score were observed between the two groups. The time of first analgesic request was earlier in the seated group (supine 131 +/- 42 min, seated 106 +/- 29 min, P = .02). Conclusion. Isobaric levobupivacaine in women at term produces a subarachnoid block the dermatomal level of which does not depend on gravitational forces.
